What You'll Do
- Travel to patients' homes, assisted living communities, memory care, and skilled nursing facilities to provide routine and PRN hospice visits.
- Complete comprehensive nursing assessments and provide expert pain and symptom management.
- Deliver wound care, medication management, and comfort-focused nursing interventions.
- Educate and support patients, caregivers, and families throughout the hospice journey.
- Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, therapists, and the interdisciplinary team.
- Complete accurate, timely EMR documentation while maintaining Medicare hospice compliance.
What You'll Bring
- Active Minnesota Registered Nurse (RN) license.
- Hospice experience preferred, or a strong background in home health, oncology, ICU, palliative care, med-surg, or another clinical setting with end-of-life care experience.
- Strong clinical assessment, communication, and critical thinking skills.
- Ability to work independently while managing a flexible field schedule.
- Reliable transportation and a valid driver's license.
