Location: Bronx, NY
Duration: 8 Weeks
Schedule: Monday – Friday | 9:00 AM – 5:00 PM
Hours: 35 Hours per Week
Compensation
Local Pay Rate: $28 – $32/hour ($1,120 – $1,280 weekly), based on experience
Position Summary
The Project Assistant will support initiatives focused on identifying and addressing patients' unmet social needs through effective screening, referral, and community resource coordination. This role will collaborate with healthcare teams, community organizations, and program stakeholders to enhance patient access to supportive services and improve program outcomes.
Responsibilities
- Assist in overseeing and improving workflows related to social needs screening and referral processes.
- Monitor program performance to help achieve established goals and quality metrics.
- Coordinate with hospital departments and community organizations to ensure patients are connected with effective support services.
- Support recruitment, onboarding, training, and supervision of Health Advocates, volunteers, and interns.
- Assist with planning and facilitating training programs, including coordinating speakers, meeting space, educational materials, and event logistics.
- Provide ongoing support and education to staff participating in social needs screening and referral initiatives.
- Collaborate with internal and external stakeholders to improve program operations and resource availability.
- Collect, review, and analyze program data to measure outcomes and identify opportunities for improvement.
- Prepare reports, presentations, and summaries for leadership and program stakeholders.
- Perform additional administrative and project-related duties as assigned.
Required QualificationsEducation
- Bachelor's Degree required.
- Master's Degree preferred.
Skills & Experience
- Knowledge of public health, community health, health policy, social determinants of health, or related fields.
- Previous supervisory or team leadership experience preferred.
- Strong written and verbal communication skills.
- Experience preparing reports, presentations, and program documentation.
- Proficiency with Microsoft Excel and other Microsoft Office applications.
- Ability to analyze data and interpret program outcomes.
- Strong organizational, problem-solving, and project coordination skills.
- Ability to work collaboratively with multidisciplinary teams and community partners.
