Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A clinical pharmacist opportunity is available for a dedicated healthcare professional specializing in home infusion therapy. This direct hire position is based in Fairport, NY, focusing on delivering expert pharmaceutical care in a home infusion setting and utilizing the CareTend system.
Schedule:
- Monday to Thursday, 8:00 AM to 5:00 PM
Key Responsibilities:
- Review infusion orders and patient therapy plans
- Participate actively in clinical rounds to assess patient progress
- Perform sterile compounding and review Total Parenteral Nutrition (TPN) formulations
- Maintain accurate patient records and individualized care plans
- Ensure compliance with USP standards and cleanroom protocols
- Provide training and ongoing support to pharmacy staff
- Lead and contribute to operational improvements within the infusion service
Qualifications:
- Current Registered Pharmacist licensure in the State of New York
- Minimum two years of recent acute care experience or at least one year of recent home infusion therapy experience preferred
- Solid understanding and experience with intravenous admixture services and IV therapy
- Proven ability to assess and monitor medication therapy in collaboration with multidisciplinary healthcare teams, ensuring optimal patient outcomes
