WCS Healthcare is partnering with a large healthcare organization to hire experienced Registered Nurses (RNs) for a Hybrid Clinical Assessment Coordinator opportunity throughout the Westchester County Region. This role combines field-based patient assessments with remote documentation and care coordination responsibilities.

Schedule & Compensation

• Full-Time
• Hybrid Schedule: 3 Days Field-Based / 2 Days Remote
• Monday-Friday
• Flexible schedules available:

Responsibilities

• Conduct initial in-home assessments for patients and members.
• Review medical records, physician referrals, and supporting clinical documentation.
• Complete and accurately enter assessments into the UAS-NY system.
• Ensure assessments comply with New York State guidelines and documentation standards.
• Identify clinical concerns, discrepancies, and care needs during evaluations.
• Educate patients and families on available healthcare services, community resources, and managed care options.
• Support enrollment into appropriate managed care programs.
• Collaborate with nurses, social workers, providers, and interdisciplinary team members to improve care coordination.
• Maintain detailed case notes and assessment documentation.
• Participate in quality assurance reviews and comply with regulatory requirements.
• Promote health maintenance, disease prevention, and continuity of care initiatives.
• Stay current with changes in assessment regulations and healthcare policies.

Qualifications

• Active New York State Registered Nurse (RN) License
• Minimum 2 years of Home Care experience
• Experience completing clinical assessments in the community preferred
• Knowledge of UAS-NY strongly preferred
• Excellent assessment, documentation, and communication skills
• Reliable transportation and ability to travel throughout the Bronx service area
