&nbsp;We are looking for a Home Care Aide to join our team! You will be directly working with patients, following a one-on-one care plan in patient homes.&nbsp; You operate with a big heart, not only providing personalized and attentive care, but by building genuine relationships with those you serve. You are detail-oriented, highly organized, and committed to creating a healthy environment that meets patient needs.&nbsp; Strong verbal and written communication skills are needed to succeed in this role. Prior Home Health and Mental Health experience is a plus. RESPONSIBILITIES 
<ul>
<li>Coverage is needed Monday through Friday (9am to 5:00pm) (11pm to 7am) and every other weekend Per-Diem (As Needed)</li>
<li>To assist the client to and from doctor's appointments</li>
<li>Maintain records of patient care, conditions, progress or problems to report and discuss observations with supervisor</li>
<li>&nbsp;Engage patients in exercises or other activities&nbsp;</li>
<li>Give medication reminders</li>
<li>Assist patients by doing laundry, cleaning the home, or assisting with personal care (if needed) and cooking&nbsp;</li>
