Job DescriptionDescription:

The Resident Care Coordinator plays a critical role in coordinating resident care and supporting the Health Services department. Working closely with the community nurse and caregiving team, the RCC helps ensure care plans are followed, changes in condition are communicated clearly, and residents receive consistent, high-quality support. This position requires prior direct-care experience in senior living, hands-on caregiving, or medication administration experience, or similar.

Key Responsibilities

  • Serve as a primary point of contact for residents and their authorized representatives regarding care coordination.
  • Communicate changes in condition and care plans to nurses, med techs, and caregiving staff.
  • Monitor and follow up on charting, care notes, and communication logs to ensure accuracy and completeness.
  • Coordinate resident appointments with physicians and outside healthcare providers.
  • Support service planning, care transitions, and ongoing care coordination
  • Assist with staffing coordination, scheduling adjustments, and coverage as needed.
  • Provide day-to-day leadership support to caregivers and medication technicians.
  • Ensure documentation and processes meet regulatory and company standards

Required Experience & Qualifications: To be considered, candidates must have:

  • Previous experience as a Medication Technician or Resident Care Coordinator in senior living
  • Hands-on experience supporting resident care in Assisted Living or a similar setting
  • Strong understanding of care workflows, documentation, and team communication
  • Ability to work comfortably with electronic health records and care management systems
  • Excellent organizational skills and attention to detail
  • Clear, compassionate communication skills with residents, caregivers, and leadership

Preferred Qualifications

  • Experience working closely with nurses or Health Services leadership
  • Familiarity with care coordination, service planning, or care transitions
  • Demonstrated leadership or lead caregiver/med tech experience
