The Care Navigation Social Worker supports referral intake and processing with a focus on psychosocial assessment, access facilitation, and one-call resolution.
WHAT YOU WILL DO:
- Assist with referral intake and processing through dispatch using multiple technology platforms.
- Support psychosocial assessment of complex and challenging referrals.
- Follow up on referrals as needed to ensure access, quality, and timeliness.
- Participate in quality improvement and process improvement initiatives.
- Participate in training peers on referral/intake/dispatch process as well as end-of-life related topics.
- Perform all other duties as assigned.
QUALIFICATIONS:
- Licensed Social Worker (LSW) in the state of Ohio
- Hospice experience preferred
- Strong multitasking abilities
- Excellent communication skills
- Ability to work effectively within an interdisciplinary team

CONDITIONS OF EMPLOYMENT:
- Compliance to Annual Flu Shot Policy or ability to provide exemption documentation
- Provide an active auto insurance policy as a licensed driver in the State of Ohio with the proper level of coverage as directed
- Provide an active driver’s license
- Ability to provide proof of eligibility to work in the United States.
- High level of integrity, ethics and professionalism.
PHYSICAL REQUIREMENTS AND WORKING CONDITIONS
This is sedentary work requiring the exertion of up to ten (10) pounds of force occasionally and some light work and lifting requiring the exertion of up to 20 pounds occasionally; work requires sitting, stooping, bending, kneeling, reaching, standing, fingering, grasping and repetitive motions; vocal communication; hearing, and visual acuity. Employee is not subject to adverse environmental conditions. This position has the potential to be exposed to environmental elements, internal and external, such as infectious disease, bloodborne pathogens, and chemical and/or chemical fumes, (pet dander and/or exposure to allergens) and adverse effects of inclement weather that may occur as well as potentially difficult patient and family situations.
