Fresh Perspective Home Care is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to provide one-on-one care for a client in the home setting. This role includes skilled nursing services and transportation assistance during schedule shifts.

Responsibilities:
- Provide direct nursing care in accordance with physician orders and the care plan
- Administer medications and treatments
- Monitor and document changes in client condition
- Communicate with supervisor regarding client needs and concerns
- Complete accurate and timely documentation
- Provide transportation assistance as needed

Requirements:
- Active LPN license in good standing
- Valid driver’s license, car insurance, and reliable transportation
- Ability to lift, transfer, and assist up to 40 lbs
- Ability to uphold client confidentiality
- Ability to work independently in a home setting
- Excellent communication and interpersonal skills
