Consultant Pharmacist (PACE Program)

$130K-$150K
Overview
We are seeking a compassionate and clinically strong Consultant Pharmacist to join our team in supporting a PACE (Program of All-Inclusive Care for the Elderly) program in Mt. Pleasant, MI. This role is ideal for a pharmacist passionate about geriatric care and interdisciplinary, patient-centered practice.
As a key member of the care team, you will help optimize medication therapy and improve health outcomes for older adults with complex medical needs.
What You'll Do
- Review and manage participant medication regimens
- Participate in interdisciplinary team (IDT) meetings and care planning
- Conduct medication reconciliations and chart reviews
- Identify and resolve medication-related issues
- Ensure compliance with CMS, state pharmacy laws, and PACE guidelines
- Educate participants, caregivers, and care team members
What We're Looking For:
- PharmD from an accredited school
- Current Pharmacist and Controlled Substance Licenses
- 2+ years of experience in geriatrics, LTC, or similar setting
- PACE experience and BCGP or Consultant Pharmacist certification preferred
- Willingness to travel
