ACT QMHP – Key Responsibilities

The ACT QMHP is an important member of Hope Network’s multidisciplinary Assertive Community Treatment (ACT) team. This role provides person-centered mental health services to individuals with significant mental health needs who may need support with daily living, safety, housing, relationships, and community participation.
The QMHP works as part of the team to assess needs, develop treatment plans, provide direct services, coordinate care, respond to crises, and support individuals in reaching their recovery goals. ACT services are available 24 hours a day, 7 days a week.
What You'll Do
- Build positive, supportive relationships with individuals served by meeting with them regularly in their homes and communities.
- Promote recovery, independence, safety, and community integration.
- Complete assessments, identify individual strengths and needs, and develop and maintain person-centered treatment plans with individuals served and the ACT team.
- Provide education and skill-building related to mental health, daily living, coping skills, and symptom management.
- Coordinate care and connect individuals with healthcare providers, families, and community resources to support needs related to housing, employment, education, benefits, transportation, and daily living.
- Support medication management and other health-related needs, as appropriate.
- Provide crisis intervention and participate in the ACT team's on-call rotation.
- Participate in team meetings, treatment planning, and collaborative decision-making.
- Perform other duties as assigned.

What You Bring
Preferred:
- Knowledge of mental health treatment, rehabilitation, and recovery practices.
- Understanding of Motivational Interviewing, Family Psychoeducation, Trauma-Informed Care, and Stages of Change preferred.
- Strong communication, organization, documentation, and teamwork skills.
- Ability to work collaboratively as part of a multidisciplinary team.
Required:
- Bachelor's degree in a related human services field or other education/credentialing that meets QMHP requirements.
- At least one year of experience working in a clinical setting serving individuals with significant mental health needs.
- Drivers license with <5 points, personal transportation, and willingness to transport persons served.
Anticipated Schedule :
- Monday - Friday 8am-4pm
- Rotating on-call requirement
