As an LPN, you will play an important role in providing quality nursing care, monitoring resident conditions, administering medications and treatments, and collaborating with residents, families, CNAs, nurses, and other members of the interdisciplinary healthcare team.
Key Responsibilities

Provide high-quality nursing care to residents according to individualized care plans

Administer medications, treatments, and physician-ordered interventions within the scope of an LPN license

Monitor resident conditions and promptly report changes to the appropriate nurse or healthcare provider
-
Supervise and coordinate resident care with CNAs and other nursing staff
-
Assist with admissions, discharges, assessments, and care planning
-
Maintain accurate and timely clinical documentation
-
Communicate effectively with residents, families, and members of the healthcare team
-
Follow facility policies, state and federal regulations, and professional nursing standards

Promote resident safety, dignity, comfort, and quality of life

Follow infection prevention and control practices
-
Provide guidance and support to CNAs and other nursing team members
-
Collaborate with physicians, RNs, therapists, and other interdisciplinary team members to support positive resident outcomes
Qualifications
Required:
-
Graduate of an accredited practical nursing program
-
Current and active LPN license in the State of Montana
-
Strong communication and interpersonal skills
-
Ability to work effectively in a team-oriented healthcare environment
-
Commitment to providing compassionate, resident-centered care
Preferred:
-
Experience in long-term care, skilled nursing, rehabilitation, post-acute care, assisted living, memory care, or geriatric nursing
-
Knowledge of medication administration, treatments, care planning, and clinical documentation
-
Experience with electronic medical records (EMR) systems
-
Previous experience working in a supervisory or charge nurse role
