EM residency graduates within the past 12 months may be considered

3-month proctorship required for applicable candidates
Candidate Requirements
-
MD or DO from an accredited medical school
-
Completion of an ACGME/AOA-approved 3-year residency in EM, FM, or IM
-
ECFMG certification required for foreign medical graduates
-
Active, unrestricted Virginia medical license
-
Active VA DEA with Virginia address
-
DEA schedules II, IIN, III, IIIN, IV, and V
Board Certification

Certifications
For providers who are not EM board certified or EM trained:

Credentialing Requirements
-
MMR and Varicella vaccines or positive immunity titers
-
TB testing within the past 12 months or QuantiFERON Gold within 3 months
-
Second TB test required before activation/privileges

Chest X-ray within 3 months if TB history or QuantiFERON is positive
-
Current-season flu vaccination
-
Case logs from the previous 2 years
-
Documentation of specialized training for special procedures, if applicable

Preeclampsia CME required initially and every 2 years for reappointment
