Job DescriptionPosition Summary
A New Horizon Inc. is seeking a compassionate, reliable, and motivated Direct Support Professional (DSP) to provide Community Development Services (CDS) to individuals with intellectual and developmental disabilities. The DSP will support individuals in becoming active, integrated members of their communities by assisting them with community participation, social engagement, skill development, and achieving person-centered goals. The DSP will promote independence, dignity, choice, and meaningful community involvement while ensuring health and safety.
Essential Duties and Responsibilities
Community Integration
- Support individuals in identifying and participating in meaningful community activities based on their interests and preferences.
- Assist individuals with attending recreational, educational, volunteer, social, and cultural activities.
- Encourage active participation and engagement in community events and programs.
- Promote relationship-building and social interactions within the community.
Skill Development
- Teach and reinforce independent living skills, communication skills, social skills, and decision-making skills.
- Assist individuals in developing self-advocacy and self-determination skills.
- Support individuals in achieving goals outlined in their Person-Centered Plan (PCP) and Service Implementation Plan (SIP).
Transportation & Community Access
- Provide transportation and travel training as authorized.
- Assist individuals with navigating community resources and public transportation.
- Ensure safe transportation practices are followed at all times.
Documentation & Compliance
- Complete accurate daily service documentation, T-Logs, and progress notes.
- Document participation, outcomes, progress, and any barriers encountered during service delivery.
- Maintain compliance with DDA regulations, agency policies, and Medicaid requirements.
- Report incidents, concerns, or health and safety issues according to agency policy.
Health & Safety
- Ensure the health, welfare, and safety of individuals supported.
- Follow emergency procedures and incident reporting requirements.
- Maintain confidentiality in accordance with HIPAA and agency standards.
Qualifications
- High School Diploma or GED required.
- Minimum 18 years of age.
- Valid driver’s license and acceptable driving record.
- Reliable transportation with proof of insurance.
- Ability to pass criminal background checks and other required screenings.
- CPR/First Aid certification (or ability to obtain upon hire).
- Experience supporting individuals with intellectual and developmental disabilities preferred.

- Ability to maintain professional boundaries and confidentiality.
- Ability to document services accurately and timely.
- Commitment to person-centered practices and community inclusion.
Physical Requirements
- Ability to walk, stand, bend, lift up to 25 pounds, and actively participate in community activities.
- Ability to transport and accompany individuals throughout the community.
Schedule
Typical CDS schedule may include:
- Monday, Wednesday, and Friday
- 9:00 AM – 3:00 PM
- Additional hours as assigned based on individual service plans.
