Role Overview
In your capacity as a Home Health LPN/LVN, you will deliver direct patient care under the oversight of a Registered Nurse, assisting in the execution of care plans to ensure patients receive safe and effective treatment in their homes.
You will be an integral member of the care team, responsible for administering treatments, monitoring patient conditions, and reinforcing education to promote successful health outcomes.
Key Responsibilities
- Administer direct patient care in line with the state Nurse Practice Act.
- Assist in implementing the care plan established by the RN.
- Provide skilled nursing services, including treatments, medication management, and wound care.
- Monitor patient conditions and communicate any changes to the RN and care team.
- Educate patients and caregivers on self-care, treatments, and disease management.
- Ensure infection control and proper usage of equipment and supplies.
- Accurately document patient care in a timely manner within the EMR system.
- Effectively communicate with physicians, nurses, and interdisciplinary team members.
- Participate in case conferences, team meetings, and ongoing training.
- Support coordinated care through consistent communication with the care team.
Qualifications and Requirements
- Active LPN/LVN license in the state of practice.
- Graduation from an accredited practical/vocational nursing program.
- Current CPR certification.
- Preferred minimum of one (1) year of nursing experience (Home Health, Med-Surg, or Acute Care is advantageous).
- Strong clinical and observational skills.
- Effective communication and patient education skills.
- Ability to adhere to care plans while identifying and reporting changes in patient conditions.
- Basic proficiency with EMR systems.
- Valid driver's license and dependable transportation.
