Job Summary
Reporting to the Chief of Growth, the Enrollment RN is a community-based position that is responsible for conducting initial assessments for potential enrollees who are interested in PACE-RI. Must be organized, courteous, and able to work independently as well as with a team. Must be able to work on multiple tasks at once, with focus and sharp attention to detail.
Duties/Responsibilities
- Schedules and conducts initial assessment including functional ability and home safety to determine eligibility.
- Presents assessments to members of the IDT bi-weekly.
- Request and interpret medical records to assist medical providers in determining course of care.
- Clearly articulate the mission of PACE while in the community to promote the program.
- Provides education on diagnoses and medical terminology to enrollment staff to guide their questions during an intake on medical conditions.
- Works with the enrollment team to determine community health needs and develop curriculum; presents curriculum and conducts clinics such as blood pressure checks while at health fairs and other community events.
- Participates in all Enrollment Strategies meetings.
- Attends IDT meetings as needed to provide information and offer solutions during the onboarding period of a new enrollees.
- Attends Provider meetings monthly to discuss new enrollees and any immediate concerns to be addressed upon enrollment.
- Conducts transition meetings with home care RN’s and day center RN’s, when applicable, to coordinate any current treatments or mediation management.
- Attends weekly enrollment department meetings to report out on the status of assessments.
- Support outreach efforts to specific referral sources as needed i.e., RN case managers, discharge planners etc.
- Always provide excellent customer service to internal and external customers.
- Recommend program improvements and solutions for greater efficiency and effectiveness in enrolling new participants.
- Participate and / or lead PACE committees that directly support Enrollment activity, or other committees established to advance the organizational strategic plan.
- Travel throughout RI is required for community-based work.
- Occasional evening and weekend hours necessary.
- Demonstrates an understanding of the importance of clear, thorough documentation as crucial to good participant care; completes and enters accurate, thorough participant documentation in the electronic health record; enters documentation for assessments, orders, and notes within required timeframes.
- Performs other related duties as required and assigned.

Education Requirements
- Associates Degree, Required
- Bachelor's Degree, Preferred
Physical Requirements
- Physical ability to regularly stoop, bend, kneel, lift, stand, walk, stretch and reach for extended periods of time.
- Must be able to regularly lift objects up to 25 pounds and at times up to 35 pounds.
- Must be able to assist a 200-250 lb. participant, using proper body mechanics.
- Must have hearing and vision abilities within normal range (corrected) to drive safely and/or to observe and communicate with participants.
- Must be able to navigate various departments of the organization's physical premises as well as related community care settings.
- Must be able to tolerate conditions typically associated within a medical office and/or home care setting including potential exposure to bloodborne pathogens and infectious diseases.
- Must be able to complete assignments in variable weather conditions and ever-changing and sometimes allergenic environments in participant homes, including extreme heat, humidity, dust, smoke, pet contamination, and cleaning supplies.
Experience
- 2 Years of direct service work (Adult Day, Nursing Home or Home Care), Preferred
- 1 Year of experience in a Medical or Geriatric Care setting, Preferred
- 1 year of experience working with a frail or elderly population, Preferred
License & Certification Requirements
- Rhode Island Registered Nurse License (RN)
Position Requirements
- Driver's License & access to reliable transportation: community-based travel required
- CPR Certification
