Activity / Activities Aide

  • Start Date: ASAP, we have a new employee orientation class starting soon
  • Hourly Wage: $16.60/hour
  • Shift: Part Time Days; 9am - 5pm
  • Schedule: 2-3 shifts per week plus an every other weekend rotation
    • NOTE: you must be able to work every other Saturday AND Sunday plus a few weekday shifts

Community Care Center is a skilled nursing facility that cares for adults who need extra support. Some of our residents are here short-term to recover after surgery or illness. Others live with us long term, including those with Alzheimer’s or memory loss. We also have a unit that cares for those suffering from mental illness. Every person here depends on us, and we take that responsibility to heart.

Job Description

When you're a patient, games, music, and engagement can be the highlight of your day. We know that emotional well-being is just as important as clinical care, and that’s where you come in.

We’re seeking an Activity Aide to involve residents in a therapeutic engagement program. You will help residents with life enrichment activities and a variety of entertainment options to help keep them engaged in their recovery programs. We are seeking candidates with a strong background in sports, culture, music, and traditional recreational activities, to help bring joy, structure, and stimulation to those in our care.

What You’ll Do

  • Lead engaging group activities, ranging from fitness and games to music and arts
  • ✅ Lead stimulating individual cognitive and memory retention events for our long-term residents
  • ✅ Contribute to programs that keep short-term patients motivated and engaged
  • ✅ Maintain accurate documentation in PointClickCare (EMR system)
  • ✅ Collaborate with interdisciplinary care teams to individualize programming

Qualifications

Activities Aide (in other centers this may be called a Life Enrichment Coordinator)

  • Prior experience as an Activity Aide in a Skilled Nursing Facility (SNF), Intermediate Care Facility (ICF), or Assisted Living Facility (ALF) is strongly preferred.
  • Candidates with related experience in recreational therapy or in providing structured, therapeutic activities for cognitively impaired or elderly populations will be considered. This includes engaging residents in meaningful life enrichment programs and leading group activities or individual one-on-one sessions.
  • The ideal candidate is outgoing, compassionate, and skilled at creating a welcoming environment that encourages resident involvement and participation.
  • Experience with electronic medical records (EMR) or patient documentation systems is preferred.
  • Must possess a high school diploma or equivalent (GED).

This center is managed by TRANSITIONAL CARE MANAGEMENT. We currently provide comprehensive management services to several inpatient healthcare centers throughout Northern IL & the Chicagoland area. These settings include Long-term Care Skilled Nursing (SNFs), Short-term Care Rehabilitation, Intermediate Care (ICFs), and Behavioral Health Service (SMHRFs) centers.
