<li> This job is for 8-hour shifts and five days each week, with the shift schedule being from 08:00 AM to 16:30 PM, a total of 40 hours per week.</li>
<li> We are looking for a Social Worker with 2 Professional References.</li>
<li> Working Days: Wed, Thurs, Fri, Sat, Sun</li>
<li> Social work license.</li>
<li> MSW</li>
<li> Discharge Planning/Crisis Intervention/Monitor Length of stay for Goals Birth &amp; Women's Unit/ED/Medical/Surgical/ICU Rotating throughout facility.</li>
</ul>
<p><strong> Roles &amp; Responsibilities:</strong>
<ul>
<li> Implement and monitor patient care plans.</li>
<li> Monitor, record and communicate patient conditions as appropriate.</li>
<li> Serve as a primary coordinator of all disciplines for well-coordinated patient care.</li>
<li> Note and carry out physician and nursing orders.</li>
<li> Assess and coordinate patient's discharge planning needs with members of the healthcare team.</li>
