This is an excellent opportunity for an experienced RN or LPN with a strong LTC background who is organized, detail-oriented, clinically knowledgeable, and interested in developing their expertise in MDS, reimbursement, resident assessments, and care planning.

WHAT YOU'LL DO

As our RN/LPN MDS Coordinator, you will play an important role in ensuring accurate and timely completion of the Resident Assessment Instrument (RAI) process, including MDS assessments, CAAs, and interdisciplinary care plans.

Responsibilities include:

  • Coordinate and oversee the RAI/MDS process, ensuring assessments are accurate, complete, and submitted on time.

  • Ensure compliance with current professional standards, physician orders, facility policies, and all applicable state and federal regulations.

  • Develop individualized resident care plans with appropriate short-term and long-term goals.

  • Collaborate with nursing, therapy, dietary, social services, physicians, residents, families, and other members of the interdisciplinary team.

  • Monitor and guide the timely completion of resident assessments.

  • Assist with and oversee the completion and transmission of MDS assessments and validation reports.

  • Schedule and facilitate interdisciplinary care plan conferences.

  • Participate in and facilitate applicable Quality Measures (QM) meetings.

  • Maintain knowledge of Medicare, Medicaid, PDPM, OBRA, CAAs, Care Plans, Case-Mix, and Quality Measures.

WHAT WE'RE LOOKING FOR

  • Active, unrestricted RN or LPN license in Tennessee required.

  • Previous Long-Term Care / Skilled Nursing experience strongly preferred.

  • Previous MDS experience is preferred, but not required for the right LTC candidate – we are willing to train!

  • Knowledge of or willingness to learn the MDS and RAI process.

  • Understanding of Medicare and Medicaid reimbursement is preferred.

  • Familiarity with PDPM, OBRA, CAAs, Care Plans, Case-Mix, and Quality Measures is a plus.

  • Strong organizational and time-management skills.

  • Excellent written and verbal communication skills.

  • Strong critical-thinking and problem-solving abilities.

  • Ability to prioritize responsibilities and meet required deadlines.
