Salary: $60-80+/hr based on experience
LaSante Health Center is seeking an Optometrist to provide comprehensive eye care services in a federally qualified health center (FQHC) setting on per diem basis. This per diem role focuses on delivering high-quality vision care while collaborating with primary care and specialty teams to support overall patient health.
Responsibilities:

Perform comprehensive eye examinations and vision assessments
</li>
<li>
Diagnose and manage common eye conditions and vision disorders
</li>
<li>
Prescribe corrective lenses and contact lenses as appropriate
</li>
<li>
Identify and refer patients with complex or surgical needs to specialists
</li>
<li>
Educate patients on eye health, preventive care, and treatment plans
</li>
<li>
Document all encounters accurately in the electronic health record (EHR)
</li>
</ul>
Qualifications:
<ul>
<li>
Doctor of Optometry (OD) degree from an accredited program
</li>
<li>
Current New York State optometry license (or eligibility)
</li>
<li>
Experience in outpatient or community health settings preferred
</li>
<li>
Strong communication and patient engagement skills
</li>
