This position will have primary responsibility for monitoring the Civil Commitment post commitment process, and back up precommitment investigations. The person in this position will have skills with engagement and community coordination of resources with a goal of stabilization into community services. Typical Duties - Duties include, but are not limited to the following Post-Commitment Monitoring Monitors all individuals under civil commitment, while inpatient and in the community. Determines necessity for and creates trial visits. Monitors all individual places on a trial visit, updates trial visits and completes revocations when necessary. Completes all court and state documents and reports. Attends court hearings. Provides consultation and training to hospitals, community members and agencies regarding the civil commitment process and procedures. Understands and works within the constraints of their role as a Post Commitment Monitor as well as maintains professional partnerships and boundaries with hospital staff and community professionals. Community Engagement Performs post commitment and investigations, as well as BRES services in the following locations: hospital and correctional environments, private residences, community organizations, and other environments as needed. Safely engages those under investigation. BRES Intake Screens all referrals to the BRES program for appropriateness. Provides intake evaluation, sets service goals for client's time in program. Coordinates use of Crisis Associates as needed for clients in BRES, with Respite Program Coordinator. BRES Case Management Connects client with community services, including but not limited to, benefits, community case management, community counseling, alcohol and drug programs, vocational resources, medical resources, and ongoing medication. Works closely with the Mental Health Specialist/ Associates at Marion County Jail and Adult Behavioral Health. Works closely with the Psychiatric Crisis Center and the Diversion Team to create alternatives to psychiatric hospitalization. Provide BRES Brief Counseling for clients on limited, as needed basis. Provide community outreach as needed. Community includes the courthouse, the annex, the jail, the mission or Simonka House, or any community location where a client may be found. Participates in diversion program meetings weekly. Provides case reviews of BRES clients. Discusses potential clients and services. Provides weekly mental health groups to participants of Lotus House. Other Job Duties Attends monthly PCC team meetings. Performs other job duties as assigned by the supervisor. <li>Assist in screening residents for discharge. Explore placement options identified by NWSDS CM and review with treatment team, resident, and legal representative.</li>
<li>For all of the above, complete clinical documentation professionally and accurately in a timely manner as directed by Marion County HHS policy. Submit paperwork to OHA/HSD or other entities as required. Maintain accurate and comprehensive MH charts. 9. Maintain a professional and supportive working relationship with clinic staff, health plan staff, administration and physicians.
10. Demonstrate high level of proficiency with documentation review including review of orders/results for lab, imaging, hospital records, EHR, etc. as a possible source for HCC codes.
11. Participate in meetings as requested in order to establish and maintain interdepartmental and external partner communication and cooperation.
12. Work with offices to coordinate completion of Annual Wellness Visits (AWV) for Medicare and Covered CA members.
13. Identifies training needs; prepares training materials and conducts coaching and training as appropriate for clinic staff, physicians and other staff to improve the quality of the diagnosis documentation and accuracy of the collection and coding of members’ health data.

The Qualified candidate is responsible for the auditing of medical documentation and codes for compliance with federal coding regulations and guidelines. This will include the assignment of ICD, Current Procedure Terminology (CPT), Healthcare Common Procedure Coding System (HCPCS) codes, modifiers, and quantities as documented by the physician. Under the direct supervision of the Manager, Utilization Management, this position is responsible for processing and coordinating specialty referrals by working closely with the referring Providers, specialists, and patients. In a Patient-Centered Medical Home, this position systematically tracks tests, referrals, and care transitions to achieve high-quality care coordination, lower costs, improve patient safety, and ensure effective communication with specialists and other providers in the medical neighborhood. The Referral Specialist collaborates very closely with each patient’s primary care provider (PCP) and care team to address any ongoing issues related to a patient’s referral.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES:
