POSITION SUMMARY
The Community Nutrition Educator (CNE) is responsible for the coordination of group and individual nutrition health education, the review of client assessments, referrals and program eligibility and the tracking of client progress in the program. The CNE also ensures data collection and entry into the charts and outcome measurement.

<ul>
<li>Adheres to funder and agency requirements for the provision of Nutrition Health Education (NHE).</li>
<li>Develops nutrition health education plans that incorporate: goals and objectives, frequency and duration of sessions, HIV specific health and nutrition topics. Performs initial processing, eligibility review, screenings and makes referrals when necessary.</li>
<li>Reviews Registered Dietician (RD) assessment and dietary recommendation to determine food service client receives and type of health education appropriate for client needs.</li>
<li>Responsible for maintaining client records and works as part of the Continuous Quality Improvement (CQI) and evaluation team for program improvement. Maintains client records that document attendance, participation, progress, accomplishments, and additional needs, referrals and goals.</li>
<li>Monitors, in conjunction with the Program Director, the appropriateness of the food service provided to the client.</li>
<li>Establishes connections and communicates frequently with Medical Nutrition Programs/Practitioners and RDs and makes referrals as necessary and appropriate.</li>
<li>Completes NHE and Food Program screening form which includes the reasons for client participation in the program.</li>
<li>Ensure clients’ adherence to all program policies, procedures, and follows program protocols for participation, voucher distribution and incentives.</li>
<li>Acts as Team Leader to conduct community coordination activities.</li>
<li>Communicates and collaborates with Funder and responds timely and appropriately to all requests from funder, Program Director and Executive Director.</li>
<li>Attend and successfully complete all required training and represent the agency at various venues promoting the program and the agency.</li>
<li>Perform other duties as assigned by the Program Director.</li>
</ul>
OTHER POSITION FUNCTIONS

<li>Serves as a liaison between Iris House and program providers and partners.</li>
<li>Demonstrates teamwork, open communication, and has good interpersonal skills.</li>
<li>Participates in in-house and community presentations, research and grant writing.</li>

POSITION REQUIREMENTS
Education:&nbsp;BA/BS or MA in Nutrition or related field such as Health Education or Family and Consumer Sciences.
Experience:&nbsp;2-3 years’ experience working in the field of HIV/AIDS or with other chronic illness; 2-3 years’ experience developing and facilitating nutrition health education.
Other Requirements:&nbsp;Bilingual (Spanish/English) strongly preferred. Must have excellent presentation and writing skills.
Possess an understanding of community level work and the importance of collaborating with other agencies.
Position requires cultural and linguistic competency with the target population.

POSITION SUMMARY

The Community Health Educator be responsible for conduct workshops to high school age adolescents, outreach to organizations that provide services to this population in Union County. He/She will be responsible for the distribution of HIV prevention education materials and testing information, record and assist in data collection and provide feedback to the Program Managers on ways to improve the effectiveness of the program and the response rate. Additionally, the Community Health Educator will be responsible for attending network events to promote the program services.
ESSENTIAL POSITION FUNCTIONS
<ol>
<li>Provide community-based education via the mental health/substance misuse through the Seeking Safety intervention &amp; HIV 101 prevention workshop to high school age adolescents in Union County in English/Spanish speaking population.</li>
<li>Engage/introduce School Administrators to program services; participate in health fairs and distribute condom packages during outreach as required.</li>
<li>Cultivate relationships with community members and gatekeepers, to effect community-level behavior change, as part of the program.</li>
<li>Present Iris House programs and services to other community based organizations, such as medical and service providers and HIV care network organizations, to cultivate and maintain linkage agreements for reciprocal services and to generate referrals for Iris House HIV/AIDS services.&nbsp;&nbsp;</li>
<li>Track and record workshops and monthly engagements with potential community partners distribution amounts; keep track on condom inventory and ensure sufficient supply to meet program needs.</li>

POSITION REQUIREMENTS
Education: Bachelor’s degree or higher in Human Services, Social Work, or related field or equivalent field experience or High School degree/GED with at least one year lived experience.
Experience: Minimum of one (1) year of outreach experience working as an HIV tester with hard-to-reach populations
