POSITION SUMMARY
The Peer Advocate will be a person with experience working with and lived experiences aligned with BIPOC (Black, Indigenous, and People of Color) individuals, with a strong focus on individuals living with HIV/AIDS, at risk for HIV, STI and Hepatic C infection and those affected by social determinants of health (SDOH), including food insecurity, housing insecurity, substance use, and/or behavioral health experiences. The Peer Advocate will conduct targeted outreach, recruit BIPOC individuals to non-medical case management services, escort clients to medical and supportive service appointments, co-facilitate support groups and determine topics for groups, assist clients with self-advocacy and self-management, and record and assist in data collection and monthly reports. Additionally, the Peer Advocate will assist in linkage to care/navigation services, making internal and external referrals, and coordinating appropriate services with program staff.
ESSENTIAL POSITION FUNCTIONS
1. Conduct target outreach and recruitment within the BIPOC population, including Ryan While eligible clients and individuals affected by social determinants of health, especially men and women of color and individuals with a history of substance use, behavioral health needs, individuals facing food insecurity and challenges with HIV medical adherence and viral load suppression.
2. Identify sites and locations to recruit participants and establish working relationships and linkages as needed. Conduct community outreach to reduce the risk of HIV/AIDS by providing HIV/AIDS education and resource information and materials. Participate in the design and preparation of activities to publicize the program within the community.
3. Assist in establishing and ensuring a friendly and safe environment during non-medical case management sessions, educational workshops, and support groups. Participate and ensure integrity of data collection and entry for evaluation purposes and participate in continuous quality improvement of program.
4. Provide educational and resource information and/or engage potential clients into Iris House programs. Provide escorting and navigation services as needed to ensure linkage to care to health providers and Iris House programs.
5. Presents Iris House programs and services to other community based organizations, such as medical and service providers and HIV care network organizations, to cultivate and maintain linkage agreements for reciprocal services and to generate referrals for Iris House HIV/AIDS services.
7. Participates in special projects and other duties as assigned by the Program Manager or other supervisory staff.
POSITION REQUIREMENTS
Education: High school degree or GED, plus 1 year experience within the Human Services or related field or equivalent educational and work experience. Non-medical case management experience preferred. Spanish Bilingual preferred.
Experience: Minimum of one (1) year of HIV and non-medical case management experience, (1) year group facilitation and outreach experience working with the BIPOC population. Lived experiences aligned with the target population.
Other Requirements: Strong group facilitation skills; Knowledge and understanding of HIV prevention, treatment and care. Excellent communication skills; must be proficient in the use of personal computers. Ability to travel extensively in New York City using public transportation. Ability to work nights and weekends as needed.
Payment: Hourly pay at a minimum of $18 per hour, two days per week w/the possibility of some additional days per week.
