The incumbent will be responsible for developing, implementing, monitoring, modifying and documenting care plans. Duties &amp; Responsibilities:

<li>Collaborates with an interdisciplinary team to identify and work toward timely resolution of issues related to psychosocial needs, including assistance with financial issues, transportation and community support services.</li>
<li>Assists in the coordination of the member's identified psychosocial needs, utilizing community resources and support when appropriate.</li>
<li>Works with external utilization management personnel and the community in identifying members who could benefit from case management services and to actualize service plans.</li>
<li>Provides social work consultation and education to members and their families and acts as an advocate as needed.</li>
<li>Analyzes assessments to identify individual needs and resources that are most appropriate to meet the member's needs.</li>
<li>Monitors all services provided to members and ensures necessary and available resources are being utilized.</li>
<li>Evaluates and anticipates members' needs, provides support, and maintains a role as the members' liaison while forecasting independence and decision-making on their part.</li>
<li>Provides referrals based on the members' assessed needs.</li>
<li>Partners and communicates with the members, family members, significant other(s), physicians and other health care providers to accomplish goals identified on the care plan.</li>
<li>Participates in proactive identification of members appropriate care management.</li>
<li>Initiates appropriate follow-up care and develops individual care management service/care plans incorporating assessment, education, resource planning and coordination of services of those patients accepted for case management.</li>
<li>Accepts referrals for case management and serves as CalOptima Health's liaison to community agencies, organizations and State of California personnel.</li>
<li>Maintains documentation of case management plans/interventions and statistics required to demonstrate the impact of case management or quality, cost effective care.</li>

Minimum Qualifications:

<li>Coordinates the planning for delivery of nursing care with the Director of Nursing and administers appropriate clinical decisions in the delivery of patient care. Proactively updates client/patients’ POC, health changes, and other related incidents.</li>
<li>Demonstrates knowledge of age-specific differences in the patient population including physiological and developmental differences unique to each group.</li>
<li>Ensures safe care to patients; adheres to all policies, procedures, and standards, including time management, supply management productivity and quality of service</li>

Patient Care Coordinators handle the clinic's clerical responsibilities of patient care. They present themselves as professionals while carrying out these duties. A Patient Care Coordinator's image and presentation to patients are a reflection both on the doctors and on our Practice. Through teamwork, patient care coordinators keep the office flow moving in a smooth and efficient manner, while maintaining a positive, respectful, and caring attitude toward patients, doctors and co-workers. 

Patient Care Coordinators should have excellent communication and organizational skills, be level-headed, dependable, courteous, compassionate, able to multitask, receptive to new ideas, ability to prioritize, self-motivator, good math and windows-based computer skills, problem solving abilities, and willing to take initiative. 

Essential Functions/Responsibilities:
All shall be performed in accordance with OSHA, DEA, HIPAA, and NPOS guidelines and expectations.

<li>Promptly and energetically greet all incoming patients and visitors</li>
<li>Answer phones promptly, with a smile, assist caller or take messages and relay to appropriate persons.</li>
<li>Schedule appointments</li>
<li>Review patient registration information for accuracy and thoroughness</li>
<li>Copy insurance card and drivers' licenses, get signature on consent, financial, and HIPAA forms, and set up charts</li>
<li>Input patient demographics into the computer</li>
<li>Note any health conditions on patient's file and in system</li>
<li>See that there is a current completed verification form for all patients</li>
<li>Collect estimated payments and co-pays from all patients at time of appointment</li>
<li>Prepare and update charts for established scheduled patients</li>
<li>When appropriate, give patients information on Care Credit options</li>
<li>Keep patients informed when schedule is running late</li>
<li>Answer basic clinical questions for patients</li>
<li>Along with surgical assistants, keep Doctors informed of any schedule changes</li>
<li>Evaluate the Appointment Schedule daily for accuracy and efficiency; make schedule changes as necessary</li>

<li>Check and straighten reception area and bathroom at end of morning and afternoon</li>
<li>Transfer to and request charts from other clinics, as necessary</li>
<li>Daily empty and distribute items from incoming couriers and prepare outgoing couriers</li>
<li>Evaluate charts for following day's patients</li>
<li>Promptly file charts and paperwork</li>
<li>Duplicate x-rays; return to referring doctor and/or give to business office if needed for insurance claims</li>
<li>Send treatment letters and referral forms to referring doctors</li>
<li>Daily turn on/off equipment</li>
<li>Be able to operate all front office equipment</li>
<li>Travel to other offices, as needed</li>
<li>Inventory office supplies and reorder through the Patient Care Coordinator Manager</li>

<li>Compiles referrals for records, by category, department office, and disposition.</li>
<li>Communicates with patients to make appointments.</li>
<li>Documents telephone call conversation when speaking with each patient in regards to the patient’s referral.</li>
<li>Responsible for following up once per week to close out referral and retrieve reports from specialist.</li>
<li>Communicates with physicians or medical offices to which the patient is being referred for medical care. Routes form to appropriate physician or office for action.</li>
<li>Meet minimum requirement of 20 referrals per day.</li>
<li>Maintain effective communication and working relationships with other employees and patients.</li>
