Marion County Health and Human Services is seeking a compassionate, energetic, and flexible Program Therapist (Mental Health Therapist 2) to work at Horizon House Residential Treatment Facility. The Program Therapist will work collaboratively within an interdisciplinary team to provide Person-Centered mental health treatment services in a 24-hour residential clinical setting.
This individual will be serving a caseload of up to 9 adult residents with serious and persistent mental illness and/or chemical dependency. Ideal candidates will have direct or related experience working in a therapeutic milieu environment, facilitating therapeutic groups, implementing/writing treatment plans, and managing doctor's orders.
The candidate will collaborate with internal and external resources to ensure the best possible treatment for residents. We actively engage our residents in treatment related activities by promoting community integration, and a recovery model approach that emphasizes independent skill development (e.g. cooking, budgeting, shopping, cleaning, nutrition, medications, social skills, communication).
Schedule is standard M-F, 0830-1700 with some flexibility based on department need. The program therapist will also maintain clinical documentation, and transport clients as needed - ODL is required.

GENERAL STATEMENT OF DUTIES Mental Health Therapist 2: Provide assessments and treatment of individuals and families, including groups; provides crisis assessment and intervention services and related duties as required. Primary responsibility of this class is to provide assessment and treatment services; crisis intervention, and treatment services, including intensive services, to individuals and their families with complex needs and diagnoses and/or to develop and implement specific services in the community.
Mental Health Therapist 1: Plans and carries out recreational, social and skills training activities; performs case management and counseling functions with Adults in Custody (AIC); facilitates counseling and/or skills training groups; performs related duties as required. Primary responsibility of this class is case management and counseling, without responsibility for developing and implementing specific programs in the community.
SUPERVISION RECEIVED Mental Health Therapist 2 and 1: Works under the supervision of a supervisor who assigns work, establishes goals, and reviews the results obtained for overall effectiveness.
SUPERVISION EXERCISED Mental Health Therapist 2 and 1: Supervision of others is not a responsibility of positions in this classification. Crisis Clinician 2 may act as Clinical Supervisor designee when needed. May assist in the supervision of practicum students and/or volunteers. 
Typical Duties - Duties include, but are not limited to the following
Provides Horizon House with clinical support that is including, but not limited to providing individual and group therapy, training staff, and communicating any clinical findings to the Horizon House staff. 
Counseling:

<li>Provide in-person individual and group therapy to individuals in residential treatment services. </li>
<li>Provide and model interventions necessary for our residents to achieve their highest level of functioning. </li>
</ul>
Community Coordination: 
<ul>
<li>Maintain current knowledge of, and contact with, community agencies in order to coordinate appropriately. </li>
<li>Coordinates with supervisor and centralized scheduler on new referrals, screenings, and intakes. </li>
<li>See that clients gain access to the services they need, and do follow-up work as needed, to make certain that clients' needs are being met.</li>
</ul>
Milieu: 
<ul>
<li>Be willing to cover an open shift or work with other supervisors to cover. </li>
<li>Be present in the milieu engaging individuals in milieu therapy. </li>
<li>Model effective milieu management for other staff. </li>
</ul>
Medication: 
<ul>
<li>Assist Horizon House team with the medication administration record (MAR) by being a backup for the weekly and monthly MAR audit, maintaining doctors' orders, coordinating with medical facilities, and communicating any medical changes with Horizon House staff/supervisors. </li>
</ul>
Hiring:

<li>Work closely with Clinical Supervisor and Program Supervisor to assist with the hiring process by participating in interview panels and assisting with reference checks as needed.</li>
</ul>
Training new hires:
<ul>
<li>Assist the clinical supervisor in training new staff in clinical services such as individual skills training, skills groups, and milieu management. </li>

EXPERIENCE AND TRAINING
<li>Possession of a Master's degree in Psychology, Social Work, Recreational Therapy, Music Therapy, Art Therapy or a behavioral science field, or possession of a bachelor's degree in Nursing and licensed by the State of Oregon, or possession of a bachelor's degree in Occupational Therapy and licensed by the State of Oregon; OR second year graduate students waivered as QMHP's by the Oregon Health Authority.</li>

The Choice Model Exceptional Needs Care Coordinator (ENCC) provides services to individuals with serious and persistent mental illnesses and complex needs, many of whom also have co-occurring substance use disorders and medical conditions. This position is an outward facing position, operates on a systems level and involves coordination with entities on the state and local levels, and involves making sound clinical decisions with complex cases.
Exceptional Needs Care Coordination 
<ul>
<li>Act as a liaison (with supervisory oversight) between ABH and various stakeholders involved in mental health care, including Oregon State Hospital (OSH), acute care hospitals, coordinated care organizations (CCO), Aging and People with Disabilities (APD), Intellectual and Developmental Disabilities Services (IDD), other programs within Marion County Health &amp; Human Services, etc. </li>
<li>Collaborate with individuals in the mental health system, partners, and stakeholders to assess individuals' needs, determine eligibility for services, appropriateness of services, authorize enrollment for Choice Model services, etc. </li>
<li>Attend OSH Interdisciplinary Treatment Team (IDT) meetings for individuals.</li>
<li>Participate in Ready to Transition determination with OSH. </li>
<li>Facilitate community transition for individuals who fall under Choice criteria. </li>
<li>Provide case specific consultation to providers and community partners, including clinical recommendations for treatment, services, and risk reduction.</li>
<li>Collaborate with acute care staff regarding care recommendations, diversion opportunities, and Long-Term Psychiatric Care needs. With clinical supervisor acting as CMHP delegate, initiate authorization process for requests for Long-Term Psychiatric Care.</li>
<li>Ensure accuracy of utilization data of those admitted to the Oregon State Hospital and acute care hospitals, provide routine reports to Marion County Management. </li>
<li>May require overnight travel to participate in coordination meetings for individuals deemed Marion County of Responsibility placed in other counties.</li>
</ul>
Licensed Care/Residential Coordination 
<ul>
<li>Assess individuals' needs, determine eligibility for services, appropriateness of services, and authorize licensed care/residential placements. </li>
<li>Identify and coordinate placements in appropriate licensed care settings, within and outside of the mental health system, for individuals discharging from OSH, acute care hospitals, and/or residing in the community. With supervisory support and oversight, negotiate with Oregon Health Authority (OHA) to advocate for Rate Reviews for complex cases. </li>
<li>Consult on LOCUS/LSI/PCSP assessments utilized in rate setting for licensed providers as needed. </li>
<li>Organize, facilitate, and/or attend treatment planning meetings for individuals enrolled in Choice Model services and collaborate with providers and agencies involved in individuals' care. Delegate such tasks to ENCC QMHA as needed. </li>
<li>Make recommendations to ABH leadership for funding for Choice Model services for licensed care placements, Supportive Housing, and other needs as identified that interfere with placement. </li>
<li>Assess individuals' readiness to step down to lower levels of care/placement when identified. </li>
<li>Ensure residential capacity, track referrals, placements, placement/discharge dates, and utilization data is being tracked by Choice team. </li>
<li>Provide clinical consultation in response to complex cases, critical and/or unusual incidents, grievances, and complaints within the residential system; this includes bi-annual training to providers on topics related to mental health.</li>
</ul>
Assessment &amp; Treatment Planning 
<ul>
<li>Complete intake and annual comprehensive mental health assessments, develop individualized treatment plans, develop crisis prevention plans, and direct provision of treatment services.</li>

<li>Participate in the development and revisions of patient specific comprehensive assessments and care plans</li>
<li>Patient and family education related to their disease process and care needs</li>
