The Lead Care Manager (LCM) role involves developing personalized care plans, coordinating member services, and collaborating closely with members and families, as well as Primary Care Providers to ensure they receive necessary medical treatment and support. Responsibilities

Qualifications

We believe that every patient deserves an incredible dentist who possesses both extensive knowledge and the ability to explain specific needs clearly and understandably. As a Patient Care Coordinator (VA Coordinator), you will play a crucial role in ensuring our patients receive comprehensive care, a smooth transition and positive experience from check-in to check-out, ensuring all pre-authorizations are approved by VA, and reviewing treatment with patients.
 
Key Responsibilities:
 
Patient Interaction
<ul>
<li>Serve as the first point of contact for patients, welcoming them warmly and addressing their needs professionally and empathetically.</li>
<li>Answer phone calls, triage issues, and handle patient requests, concerns, and complaints in a friendly and efficient manner.</li>
<li>Discuss treatment plans, estimated insurance coverage, and patient balances clearly with patients.</li>
</ul>
Scheduling and Coordination
<ul>
<li>Schedule appointments and manage the daily schedule to optimize efficiency and capacity.</li>
<li>Ensure all patients are scheduled for follow-up appointments at the end of their visit.</li>
<li>Work on continuing care, hygiene recall, and overdue treatment lists.</li>
<li>Create and send pre-authorizations to VA Community Care for all veteran patients.</li>

Payment Handling:
<ul>
<li>Determine estimated patient co-pays, deductibles, and other expenses prior to appointments and document them accurately.</li>
<li>Accept and record payments made at the time of service, via mail, and over the telephone.</li>
<li>Present payment options to patients with third-party services and make necessary arrangements according to practice protocols.</li>
</ul>
Pre-Appointment Preparation:
<ul>
<li>Ensure all necessary radiographs, lab cases, paperwork, and other information are updated and complete prior to patient appointments.</li>
<li>Verify that medical clearances are received if needed before appointments.</li>
<li>Ensure all insurance is verified prior to appointments.</li>
<li>Verify all VA patients have proper approvals prior to appointment date and time.</li>
</ul>
Administrative Support:
<ul>
<li>Send requested records to patients and update patient files.</li>
<li>Check in lab cases and document appointments with pertinent information.</li>
<li>Maintain active communication with VA Community Care in regard to patients’ appointment date/times as well as next steps in treatment.</li>
<li>Perform any other duties as assigned.</li>
</ul>
Qualifications:
<ul>
<li>Proficiency with Denticon software is required.</li>
<li>Strong phone and scheduling skills.</li>
<li>Dental Experience REQURIED</li>
<li>Dental Insurance knowledge is required.</li>
<li>Excellent communication and interpersonal skills.</li>
<li>Ability to manage multiple tasks efficiently and effectively.</li>
