Job Summary
The Behavioral Health (BH) Patient Services Coordinator plays a vital role in supporting outpatient behavioral health care for established and incoming patients who need psychiatric or counseling services. This individual ensures a welcoming, organized, and compassion environment and links patients to his/her BH provider, assists with administrative and clerical duties, and supports patient behavioral health needs. The coordinator helps connect patients to services and resources while always maintaining confidentiality and professionalism. This is a position of trust and one of frontline support within our care team.
Communication
Good oral and written communication skills are required for this position. Information must be exchanged professionally and appropriately as well as documented in the patient care record. The person in this position must be able to communicate with the patients, staff, and others and to contribute to a positive BH healthcare environment. The ability to communicate in Spanish, Creole, or both is a plus but not required.
Key Responsibilities

<li>Assist BH psychiatric treatment services when scheduled including measuring and recording patient vitals (blood pressure, pulse, temperature, weight, etc.) per clinic protocols and ensure all vitals are entered accurately into the EMR and communicated to the care team in a timely manner.</li>
<li>During treatment days, observe patients for any signs of distress or immediate behavioral health concerns and report findings to the BH psychiatric provider promptly.</li>

A Day in the Life...
As the Business Services Relationship Manager, your typical day may include the following:
<ul>
<li>As you sip your morning coffee, you log in reviewing your calendar to prepare call plans for the COI's, prospects, and existing members you lined up from the previous week.</li>
<li>You attend the morning huddle with your department to discuss the weekly workload, plans for success or gain support for any hurdles you face. </li>
<li>Following the review of the pipeline, you begin digging into current deals. Updates are communicated to Credit and your department to ensure quality and transparency. </li>
<li>You take the time to follow up with current borrowers and handle any requests to move the deal forward, or handle action items. </li>
<li>Now it's time to make calls or join pre-scheduled zoom meetings to new prospects or Center of Influences, knowing you were prepared by creating a call plan pre-meeting. During the call you take notes, identify opportunities, and continue building relationships. As the call wraps up, you log your lead and notes, capturing the discussion, setting the stage for additional follow up calls. </li>
<li>You switch gears to prospecting new relationships making outbound calls or emails to new COI's and prospects. Networking through Facebook, LinkedIn, or other sources, logging your efforts.</li>
<li>Following prospecting, you make calls to current members as a retention and relationship strengthening effort. </li>
<li>It is now time for your personal development. In this role you report directly to the Lending Sales Manager where you have a scheduled meeting discussing measures of success, or simply going over your needs for future growth. </li>
<li>As the afternoon turns to 2pm, you finished lunch and fly from the office to meet a potential borrower for coffee. Upon return to the office, you log the call, send a thank you email and set up the next meeting. </li>
<li>Taking an hour after you return from coffee, you review emails and conduct follow up on any action items that came from the working day. </li>

The American Lung Association has an excellent opportunity for a Manager, Health Promotions. Working as a member of the Health Promotion &amp; Education department, you will join an outstanding group of professionals dedicated to our mission: to save lives by improving lung health and preventing lung disease through research, education, and advocacy. 
Manage and implement Addressing the Burden of Childhood Asthma in New York State grant including evidence-based strategies to expand the quality and availability of guidelines-based asthma control services. Efforts aimed at improving asthma-related health outcomes and quality of life among children with asthma and their families/caregivers across New York’s highest asthma burden areas will be driven by the strategies outlined in the Center for Disease Control (CDC) National Asthma Control Program’s EXHALE Technical Package. This is a 100% grant-funded position serving the State of New York, focusing primarily Brooklyn, Queens, Manhattan, and the Bronx.
Location: This position is located at the American Lung Association's New York, NY office and will be a hybrid of in-person and virtual work. 
Responsibilities: 
<ul>
<li>Design, implement, evaluate, and monitor evidence-based initiatives for the Addressing the Burden of Childhood Asthma in NYS grant by establishing and managing work plan projects that address the needs of the high-risk population incorporating the EXHALE Technical Package.</li>
<li>Manage and deliver Project BREATHE NY to members of the multidisciplinary asthma care team across many healthcare settings. Ensure program quality, performance and effectiveness.</li>
<li>Develop, maintain, and disseminate asthma care delivery tools to guide implementation of guidelines-based care.</li>
<li>Collaborate with other key staff members, partners, and organizations working to expand comprehensive asthma control services.</li>
<li>Work with national and local advocacy team to promote asthma as a priority.</li>
<li>Develop and submit timely reports as required by the association and funding sources, including monthly management reports, grant reports and program metrics reports. </li>
<li>Present program outcomes and disseminate surveillance/ evaluation findings at statewide and national meetings as needed.</li>
<li>Represent the American Lung Association as a member of community, civic, and/or health coalitions and organizations related to the Association’s mission.</li>
<li>Build, maintain, and cultivate relationships with funders, facilitators, and community partners to encourage program delivery, sustainability, and growth across service territory, including American Lung Association programs where applicable. </li>
<li>Select and participate in local, state, and national seminars and courses designed to increase skills and knowledge related to job requirements.</li>
<li>Participate in and support all area events and provide support for annual report, awards, grants and other activities as assigned. </li>

Qualifications:

<li>Bachelor’s degree in public health, Health Promotion, or related field required. Master’s degree in public health, or a related field preferred. </li>

<li>Certified Asthma Educator (AE-C), Respiratory Therapist (RRT), Registered Nurse license (RN) or Certified Health Education Specialist (CHES) preferred; must obtain AE-C within one year of hire. Prior experience in public health, social services, public policy, and/or advocacy required.</li>
<li>Experience in coalition building, developing and implementing community awareness, education, and programs specifically related to areas of public health. </li>
<li>Experience in the management, implementation and evaluation of programs. </li>
<li>Must be a self-starter with excellent communication skills, both written and oral.</li>
<li>Positive attitude with the ability to work independently and cooperatively in a team environment.</li>
