Responsibilities
•&nbsp;&nbsp; Complete evaluations, assessments, and treatments in accordance with policy and the individualized physical therapy care plan
•&nbsp;&nbsp; Complete all documentation and records in a timely and accurate manner
•&nbsp;&nbsp; Follow established policies and procedures as related to the scope of practice
•&nbsp;&nbsp; Recognize and respond to priorities in patient care showing initiative and sound judgment
•&nbsp;&nbsp; Complete all required orientations, in-services, and trainings successfully and in a timely manner

LICENSURE, CERTIFICATION AND EDUCATIONAL REQUIREMENTS
•&nbsp;&nbsp; Valid professional license in the state of practice
•&nbsp;&nbsp; Graduate of CAPTE accredited school.&nbsp; If foreign-trained, the program must be deemed substantially equivalent to a Physical Therapist entry level education in the U.S. by a credential’s evaluation organization approved by the APTA

Physical Therapist
JOB SUMMARY
Plans, organizes, and implements Physical Therapy services for PACE participants. Responsibilities include but are not limited to assessment; planning, providing treatment; and teaching participant, caregiver or other appropriate representatives/family to maintain participant support in the community.&nbsp;
ESSENTIAL DUTIES AND RESPONSIBILITIES:
<ul>
<li>Provides high quality care and serves as a member of the PACE interdisciplinary team (IDT).</li>
</ul>
<ul>
<li>Performs in-person assessments for participants at initial enrollment and as-needed ongoing as determined by the IDT. &nbsp;</li>
<li>Assesses the need for adaptive durable medical equipment, specialized shoes and prosthesis. Ability to assess participant’s home for safety and mobility. Recommend home modifications and equipment.&nbsp;</li>
<li>Plans for physical therapy treatment programs according to recognized standards and policies.&nbsp;</li>
<li>Administers physical therapy treatment programs consistent in achieving identified goals and according to recognized standards and policies. Confer with interdisciplinary team on participant care issues.&nbsp;</li>
<li>Documents evaluation, assessment, plan and treatment rendered accurately and in timely according to policy.&nbsp;</li>
<li>Measures effectiveness of treatment, document and revise plan accordingly.&nbsp;</li>
<li>Educates and instructs participants/family in procedures, home programs and document training and understanding by participant/family.&nbsp;</li>
<li>Trains PACE staff in body mechanics, use of equipment, transfers, gait patterns, and methods to maximize safety and functional abilities in performing activities of daily living (ADL’s).&nbsp;</li>
<li>Provides care and services in the center, clinic, contracted facilities, and participants’ homes according to each participant’s plan of care.&nbsp;</li>
<li>Assists participant to function at most independent level possible.&nbsp;</li>
<li>Develops and maintains positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.</li>
<li>Assists the interdisciplinary team members in understanding the significant rehabilitation needs of the participant and families&nbsp;</li>
<li>Collaborates with the interdisciplinary team to develop a comprehensive care plan for each participant.</li>
<li>Maintains regular attendance at and participates in daily Interdisciplinary Team meetings, communicates participant changes and collaborates with team members in care planning decisions and coordination for 24-hour care delivery.</li>
<li>Performs the duties of other IDT members based on professional licensing, competencies, and experience as needed.</li>
<li>Actively participates in quality improvement projects and meetings. Fulfills the role of other IDT member(s) as needed according to licensure and competency requirements.</li>
<li>Attends and actively participates in a variety of organizational meetings related to participant care or daily operations, in-services and community agency meetings.</li>
<li>Completes all documentation of clinical service in participants’ medical records, including initial assessments; reassessments; change of status; and other significant events according to PACE documentation requirements.</li>
<li>Follows all Policies and Procedures and OSHA safety guidelines.</li>
<li>Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants, and families.</li>
<li>Practices standard precautions.</li>
<li>Maintains a safe working environment, following safety policies and procedures.</li>
<li>Participate in continuing education classes and any required staff and training meetings.</li>
<li>Maintains professional affiliations, required certifications and continuing education requirements.</li>
<li>Other duties as assigned.</li>

EXPERIENCE EDUCATION AND CERTIFICATIONS:
<ul>
<li>Bilingual and fluent in Hmong</li>
<li>Advanced degree in Physical Therapy from an accredited school of Physical Therapy</li>
<li>Current license to practice Physical Therapy in the state</li>
<li>Current driver’s license and proof of auto insurance</li>
<li>Two (2) years of experience working on an interdisciplinary team in a hospital, nursing home or community-based setting is preferable.</li>
<li>1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role). &nbsp;</li>

We are a small physical therapy clinic looking for an EXCEPTIONAL person to join our team and help make a difference. We are looking for a highly motivated, team oriented PT who can also work autonomously. Our outpatient physical therapy clinic is highly regarded in the community from both patients and referring doctors.
 
We are a therapist owned clinic which values each employees needs.
We are in network with most insurances and Medicare. Typical client is 25-65 years old, but we have seen as young as early teens and clients into their 80's. We tend to mostly see an active population. We do not see many workers compensation. We do not take leins. 
 
Most of the therapist here are manual therapist with a heavy emphasis on exercise prescription to follow it up.
 
We are somewhat flexible on hours, looking to cover:
Monday to Thursday, 4x10 hour starting around 7am or possibly Monday through Friday 10am 7pm, (Most likely 6 on Friday's).
