Duties and Responsibilities
<ul>
<li>Deliver excellent customer service by making and receiving inbound and outbound calls related to medication delivery</li>
<li>Review notes before confirming delivery, and communicate with patients or physicians regarding insurance investigations and patient responsibilities</li>
<li>Collect key clinical data via URAC patient assessments</li>
<li>Ensure accurate scheduling for medication deliveries, medication storage instructions, new patient packets, and refill procedures</li>
<li>Collect applicable co-payments from patients during scheduling (credit card, debit card, etc.)</li>
<li>Process credit and debit card payments for all major medical and self-pay patients</li>
<li>Schedule timely deliveries of medication and nursing services for AIC and home infusion patients to ensure compliance with treatment plans</li>
<li>Document all necessary information in the appropriate system, ensuring the correct ship date and delivery address are recorded</li>
<li>Verify the accuracy of patient charts, including addresses, medication shipping details, and insurance authorization</li>
<li>Notify RPH/RN of any patient side effects or requests</li>
<li>Resolve issues promptly and document all relevant information on patient records</li>
<li>Report any misconduct, suspicious, or unethical activities to the Compliance Officer</li>

Responsibilities

What You Will Do:The role of the Patient Care Coordinator is to serve as a liaison between the Clinical staff and the patient to coordinate expenses, clinical care, and administrative needs. The Patient Care Coordinator collaborates with the Nurse Coordinators and takes daily direction from the Nurse Manager.

<li>Track patient workup to ensure current preconceptual labs, communicable testing, pap smear, annual exam, and mammogram (if applicable) are complete.</li>

<li>Educate new patients after initial consultation with physician, prior to discharge, and refer to the primary nurse if immediate cycling is anticipated.</li>
