Description of Responsibilities
The Home Health Physical Therapist provides skilled physical therapy services to patients in their homes on an intermittent visit basis. This role is responsible for assessing, planning, implementing, and evaluating physical therapy care to restore function, improve mobility, relieve pain, and prevent or limit permanent physical disabilities.
