As a member of the behavioral health team at Community Health Centers of Greater Dayton (CHCGD), this position will work collaboratively with the Psychiatrist, Behavioral Health Director, Behavioral Health Consultants, Primary Care Physicians, and other administrative and operational staff to ensure continuity of care for patients meeting criteria for mental health, substance use, and other related psychiatric illnesses. The CHCGD organization currently has six site locations and services can be provided as office-based services and telehealth services as well.
Principal Duties and Responsibilities
· Provides psychiatric evaluations and medication management services
· Provides diagnosis and medication education to patients and their families
· Provides Behavioral Health consultations to Primary Care Providers and other member of the Behavioral Health team as needed
· Facilitates the hospitalization of patients served who require inpatient care
