You will be a resource for high-risk pregnancy members, delegated plan Case Managers, Personal Care Coordinators (PCCs) and community partners to address medical, behavior and psychosocial concerns. In conjunction with other team members, you will develop and make recommendations for a comprehensive individualized care plan at all levels of care. <li>Works collaboratively with the PCCs to develop care plans for members, including those who are pregnant, using member HRA data.</li>
<li>Utilizes all other available data to formulate care plans in the absence of an HRA.</li>
<li>Ensures and facilitates communication between PCCs and delegated plan PCCs to achieve favorable member health outcomes.</li>
<li>Responds appropriately to case management triggers generated as part of the HRA or ICT process.</li>
<li>Anticipates needs for those at risk of transitioning of care and proactively support reducing the risk of re-entering a higher level of care.</li>
<li>Acts as an integral part of the team, providing verbal and written support of member needs.</li>
<li>Collaborates and communicates with the member, family, significant other(s), physicians, health network (HN) designee and other health care providers to support and accomplish goals identified on the individualized care plan in all care settings.</li>
<li>Accepts referrals for case management and serves as CalOptima Health's liaison to community agencies, organizations and state personnel.</li>
<li>Provides case management consultation and education to members, their families and the HNs.</li>
<li>Acts as an advocate to assist in the coordination of the client's identified psychosocial needs, utilizing community resources and support when appropriate.</li>
<li>Participates in proactive identification of patients appropriate for care management.</li>
<li>Initiates appropriate follow-up and the development of an individual care plan incorporating assessment, education, resource planning and coordination of services of those patients accepted for case management.</li>
<li>Assists, as requested, to collect and analyze data from initial/annual/updated health risk assessments, stratification tools, outcome measures and internal/external reports to identify trends, individual issues, problems and resources that are most appropriate to meet client's needs.</li>
<li>Develops and maintains a network of current community resources and services where members can be referred for assistance.</li>
<li>Performs regular review of cases and events to identify patterns/trends and to ensure the department's compliance with CMS, Department of Health Care Services (DHCS) and National Committee for Quality Assurance (NCQA) standards.</li>
<li>Maintains documentation of case management plans/interventions and statistics required to demonstrate activities of the ICT and the impact of case management, quality and care settings that best meet the member's needs.</li>

Medical Case Manager (LVN) (High Risk Pregnancy)

The Chief of Endocrinology will provide strategic leadership and direction for all aspects of the&nbsp;Division. The Division Chief will be responsible for all planning, policy, operations, and&nbsp;oversight of academic, research, and clinical programs of the Division. The Chief of Endocrinology&nbsp;will have financial authority over the division&nbsp; budget, including extramural research and&nbsp;philanthropic funding received and other financial matters for the division, in accordance with&nbsp;applicable University policies and practices. The Chief of Endocrinology will have discretion in&nbsp;the recruitment and retention of division faculty who will have a direct reporting line. Qualified&nbsp;candidates must possess a Doctor of Medicine (MD), Doctor of Osteopathic (DO) or equivalent degree&nbsp;from an accredited institution and be eligible for a University of Arizona academic appointment at&nbsp;the Associate or Full Professor rank. The Chief of Endocrinology will also play a prominent role in&nbsp;the establishment of a Diabetes Research Center with the support of an ongoing University of&nbsp;Arizona (UA) capital campaign and the continued development of an NIH - T32 training program in&nbsp;diabetes, endocrine, and metabolism.
The Endocrinology division is one of twelve within the Department of Medicine, which leads the&nbsp;University of Arizona, College of Medicine-Tucson in research funding, and has two residency&nbsp;programs, 15 fellowship programs including endocrinology, and numerous mentorship opportunities in&nbsp;multidisciplinary health science graduate programs. <li>Develops financial, growth, satisfaction, and clinical metrics to reflect performance of the department.&nbsp; </li>
<li>Directs and participates in special projects that promote the growth, efficiency, and operations of the program.&nbsp; </li>
<li>Collaborates with the multi-disciplinary team to develop operational and strategic goals and objectives consistent with the National Association of Epilepsy Centers – Level 4 and the DNV or Joint Commission Disease Specific Certification in Pediatric Epilepsy.&nbsp; </li>
<li>Works in a dyad leadership role with the Epilepsy Medical Director to coordinate the development, implementation, review, and revision of Epilepsy Program standards, policies, procedures, and protocols based on current clinical practice guidelines. </li>
<li>Co-leads the Epilepsy Leadership and operations meetings including a) agenda &amp; minutes, b) reports for clinical metrics/core measures, c) actions, timeline, and responsible person.&nbsp; </li>
<li>Contributes to and supports execution of the Center and Institute's Strategic Plan.&nbsp; </li>
<li>Maintains a clinical dashboard including outcomes metrics in the clinical, functional/QOL, and perception of care domains.&nbsp; </li>
<li>Identify trends and patterns and opportunities for improvement, maintaining two QI initiatives ongoing. </li>
<li>Supportive to the financial dashboard and improvements against targets.&nbsp; </li>
<li>Reviews all epilepsy related order sets annually/on-going with Medical Directors, Nurse Navigator, and epilepsy committee.&nbsp; </li>
<li>Monitors compliance with epilepsy metrics and scorecard.&nbsp; </li>
<li>Monitors compliance with national epilepsy regulations and guidelines.&nbsp; </li>
<li>Identify and participates in performance improvement initiatives.&nbsp; </li>
<li>Partners with Clinical Education to identify and address educational needs for staff around topics like target metrics and new guidelines/treatment. </li>
<li>Rounds in epilepsy units with Unit Manager and collaborates regularly with the ICU Manager, Neurodiagnostic Manager, Radiology leadership, and Ambulatory, and Clinic leadership.&nbsp; </li>
<li>Ensures full collaboration with Research and Medical Directors for desired outcomes with the National Registry Data like PERC, Epilepsy Learning Health System (ELHS), PedsQL or selected QOL survey(s), The National LAANTERN Registry, NIH RO1 Grant studies, multicenter studies, and all epilepsy research.&nbsp; </li>
<li>Supportive and engaged with COE presentations, publications, and talks.&nbsp; </li>
<li>Accountable for U.S. News and World Report Neurology metric reporting and outcomes to achieve top ranking.&nbsp;</li>

Qualifications
Minimum Job Requirements
<ul>
<li>Master's Degree in Nursing or Healthcare related field </li>
<li>Registered Nurse within the State of Florida or Multi-State Enhanced Nursing License Compact (eNLC) – maintain active and in good standing throughout employment </li>
<li>3-5 years of clinical experience with the neuroscience population</li>
</ul>
Knowledge, Skills, and Abilities
<ul>
<li>Lean certification and experience preferred. </li>
<li>Proficient in Microsoft Office (Word, Excel, and PowerPoint) </li>
