The Personal Care Assistant is a vital part of our team, providing aid to our participants as well as contributing to the care planning process as a member of our interdisciplinary team (IDT). On the day-to-day, you will:
<ul>
<li>Provide hands-on personal care assistance with activities of daily living (ADLs) to WelbeHealth participants, including but not limited to assisting with escorting, toileting, bathing, dressing, grooming, laundry, and physical lifting when transferring participants</li>
<li>Measure and document participants' vital signs as instructed</li>
<li>Act as a medical escort when needed, assisting participants with boarding and exiting WelbeHealth transportation vehicles</li>

Position Overview: We are seeking a skilled and compassionate Nurse Practitioner (NP) or Physician Assistant (PA) to provide patient-centered, trauma-informed primary care as part of a multidisciplinary team. This role focuses on addressing acute and chronic health conditions, promoting health equity, and delivering holistic care to individuals facing complex social and health challenges. 
The Advanced Practice Provider (APP) provides quality primary healthcare services to special populations, emphasizing the promotion of health and the prevention of disease. This role addresses society's demand for accessible and affordable healthcare. The APP works as an integral member of a multidisciplinary team, managing acute and chronic health issues while collaborating with physicians, nurses, and other healthcare professionals. Individual accountabilities and work volumes will be established annually.
Essential Duties and Responsibilities:

Qualifications:
Education and Experience:
• Physician Assistants (PAs):

<li>Active Illinois PA licensure required.</li>
<li>Graduation from an ARC-PA-accredited program.</li>

• Nurse Practitioners (NPs):
