POSITION PURPOSE:&nbsp;Advanced Practice Clinicians (APC) provide patient-centered, personalized health care to clients. APCs along with Health Center support staff create a safe, non judgmental environment for people of all genders to access high quality reproductive &amp; sexual healthcare.&nbsp;&nbsp;
<h3>YOUR DAY- TO-DAY RESPONSIBILITIES:</h3>
<ul>
<li>Provides exceptional patient-centered care, in a manner that ensures compliance with current PPNNE efficiency and productivity standards.</li>
<li>Assesses patient's health status and prescribes appropriate contraceptive methods, treatments for GYN problems</li>
<li>Counsels/educates patients regarding general reproductive health care; all FDA-approved methods of birth control, including emergency contraception; pregnancy options including abortion, adoption as well as Gender Affirming Hormone Therapy</li>
<li>Provides health care that is culturally and linguistically appropriate to PPNNE patient populations.</li>
<li>Performs appropriate physical exams, with focus on the reproductive system, including standard initial and annual exams, and screening procedures</li>
<li>Refers patients to physicians and other health or social services as appropriate</li>
<li>Responsible for accurately coding for medical services provided, in support of the organization’s overall revenue cycle.</li>

We provide Mammograms, Dexa Scan, and have lab &amp; ultrasound available on site.
Our midwifery service provides care to a highly educated and motivated population.
The position is located at our Fort Collins office, serving the Poudre Valley Hospital.&nbsp;Two years full-scope experience is preferred, eligible for full-state licensing and DEA prescriptive authority and C-Section first assist experience is beneficial.
The position requires a rotating schedule of 1 call and 2 clinics per week or 2 calls and 1 clinic per week.&nbsp;All weekday calls are 24hour call shifts and weekends are rotated at 48 hours per weekend on a five week rotation.&nbsp;We have a very supportive MD hospitalist back up team. The CNM will provide direct antepartum, intrapartum, postpartum, OB triage, and gynecologic care to patients in accordance with standards established by the American College of Nurse-Midwives. Income is based on a productivity formula that pays for Clinical RVUs and a per diem amount for each call day and has no cap. Our current midwives average 100,000 to 130,000 per year depending on that formula.

They need to have also passed the national certification exam by the American Midwifery Certification Board (AMCB). What is the purpose of this role?
The Certified Nurse Midwife (CNM) will bring compassionate, in-home prenatal care to women and families who often face the greatest barriers to accessing healthcare. As one of the first midwives in Nest’s new prenatal program, this role is an opportunity to provide hands-on care and build trusting relationships with moms through pregnancy, while also shaping how Nest delivers this vital service in the future. The CNM will walk alongside families up to 36 weeks of pregnancy, then help ensure a smooth transition to delivery and back into Nest’s family-centered care after birth.
What is the role?
The Certified Nurse Midwife will spend most of their time in the field delivering in-home prenatal visits, monitoring maternal and fetal health, providing patient education, and ensuring smooth transitions of care. They will work closely with Nest’s interdisciplinary team to integrate prenatal care with other primary, behavioral, and social services. While their primary focus is direct care, the CNM will also provide practical feedback as an early adopter of this program, helping ensure the model best meets the needs of patients and families.
What will you do?

Deliver in-home prenatal care through 36 weeks gestation, including routine assessments, counseling, and management of common pregnancy-related conditions.
</li>
<li>
Provide culturally responsive, trauma-informed, patient-centered care to Medicaid patients.
</li>
<li>
Educate and support expectant mothers and their families throughout pregnancy.
</li>
</ul>
<ul>
<li>
Participate in an on-call rotation for prenatal patient concerns, with calls first triaged by 24/7 RN line and escalated to the CNM as appropriate.
</li>
</ul>
<ul>
<li>
Coordinate care transitions at 36 weeks to delivering providers/facilities, and back to Nest for postpartum/dyad care after delivery.
</li>
</ul>
<ul>
<li>
Collaborate with Nest’s interdisciplinary team to ensure seamless integration of prenatal care into the family care model. Aim to close care gaps and improve maternal-child health outcomes for expecting mothers.

Share insights and feedback from patient care experiences to support continuous improvement of the prenatal program.

What do you bring to the Nest?

Current certification as a Certified Nurse Midwife (CNM) with active licensure (or eligibility) in [insert state].
