BAYADA Home Health Care is currently seeking a Transitional Care Manager (TCM) to join our Cape May Community Sales Team. Clinical Liaisons / TCMs at BAYADA interact with discharge planners, case managers, and clients and families at designated facilities to coordinate the initiation of home health care services. 
Territory: Cape May County
The TCM will:
<ul>
<li>Develop relationships, in accordance with The BAYADA Way, with referral sources, physicians and other health care professionals.</li>
<li>Market and sell BAYADA services by clearly articulating the features benefits of BAYADA.</li>
<li>Demonstrate mastery of clinical and regulatory home care eligibility criteria.</li>
<li>Review all medical documentation and work with physicians to make clinical judgments about clients’ eligibility for home care services and if BAYADA has the resources to provide the ordered services.</li>
<li>Coordinate with the service office to set up the services ordered by the physician.</li>
<li>Complete weekly referral log.</li>
</ul>

Qualifications for a TCM:
<ul>
<li>4 year degree, preferred</li>
<li>Demonstrated record of strong interpersonal skills.</li>
<li>Demonstrated knowledge of the Medicare COPs and other program requirements, as applicable.</li>
<li>Previous discharge planning experience</li>

Compensation

The role of the Home Health &amp; Hospice Care Navigator is to drive post-acute skilled Home Health service census results for the Bellefontaine/Marysville/Dublin/New Albany Ohio areas. The position will work with the Otterbein Small House Neighborhoods, Otterbein SeniorLife Communities (IL/AL/SNF), local skilled nursing facilities, and hospital systems. Additional duties may include participation in the Chambers of Commerce, social/business groups, senior centers and churches in the assigned territory. This position is full time, 40 hours per week. 
Responsibilities
<ul>
<li>Establish and fully develop relationships with assigned contacts (administrators, doctors, social workers, and discharge planners) to drive referrals for patients needing skilled home care services.</li>
<li>Ensure financial viability of the local ministries by developing and maintaining a relationship that yields an increased share of patient referrals from the targeted locations.</li>
<li>Monitor and maintain targeted census to meet financial goals and assure financial stability for the ministry.</li>
<li>Assist in the development and implementation of the marketing plans for SW Ohio.</li>
<li>Assess potential referrals or potential admissions and work to admit the patient to Otterbein Home Health. This will include gathering the required orders, documentation and communicating with physicians to obtain signatures as needed. </li>
<li>Distribute brochures, fliers and other promotional items to referral sources in the targeted areas.</li>
<li>Work closely and collaboratively with the community and Otterbein team members to ensure an easy transition of patient onto Otterbein Home Health services.</li>
<li>Coordinate marketing efforts to meet census goals and to keep the Otterbein location staff advised of anticipated referrals and/or admissions.</li>
<li>Identify opportunities and execute those selected opportunities to make working with Otterbein easier and more efficient for referral source.</li>
<li>Performs any other duties as assigned.</li>
