Position Summary
Reporting to the chief medical officer (CMO), the advanced practice nurse in collaboration with Ocean Health Initiatives, Inc. (OHI), staff physicians, will provide primary care services to the patients of OHI that is consistent with established policies, procedures and protocols, OSHA standards, and other safety and health related codes and laws.
Responsibilities
<ul>
<li>Coordinates and manages individual patient care in collaboration with the chief medical officer and staff physicians.</li>
<li>Works cooperatively with all providers and staff to carry out the goals and objectives of OHI, meets productivity according to established policies and procedures.</li>
<li>Provides continuing care to assigned patients in the office setting, anticipating and recognizing deviations from normal, initiating treatment, and consulting with a physician as appropriate. </li>
<li>Orders diagnostic tests, medications and lab tests as indicated. Analyzes, interprets and synthesizes data and applies information to patient management. </li>
<li>Provides education and counseling for patients, families, and community groups on various health conditions.</li>
<li>Functions as a member of a multi-disciplinary health care team.</li>
<li>Assists with the development, implementation, and monitoring of clinic policies, procedures and protocols, as needed to achieve OHI goals and objectives.</li>
<li>Assists with the development, implementation, and monitoring of OHI performance improvement program.</li>
<li>Accountable for encounter productivity, quality measures and family planning.</li>
<li>Attends and actively participates in departmental and medical staff meetings as required.</li>
<li>Supports health care center colleagues with internal referrals when clinically appropriate.</li>
<li>Collects patient information and records patient information, such as medical history and reports examination results. </li>
<li>Ensures all patient charts and justifies prescribed treatment and includes findings/progress, and plan of care, in accordance with OHI policy and procedures.</li>
<li>Provides medical care for all individuals without regard to ability to pay, race, color, origin or creed, according to OHI policy.</li>
<li>Provides consultation, guidance and directions to medical support staff to ensure continuity and quality of care services.</li>
<li>Effectively, efficiently and timely completes all paperwork requirements for billing and medical records compliance.</li>
<li>Advises patients on good health practices.</li>
<li>Uses effective clinical judgment in referring patients and receiving referrals. Refers patients to specialists when required and in accordance with their insurance restrictions.</li>
<li>Develops and implements goals and objectives to ensure high quality, cost effective clinical care.</li>
<li>Ensures all patient charts are completed within 24 hours of the patient visit and the charting is accurate to support clinical and documentation requirements via electronic medical records (EMR).</li>
<li>Ensures that all labs, imaging or other items in the buckets are addressed and closed in accordance with OHI policy and procedures. </li>
<li>Plans and implements policies, procedures, and systems to achieve stated goals and to ensure compliance with regulatory/third party mandates.</li>
<li>Active participant in OHI's performance improvement process, departmental meetings, and other OHI meetings as scheduled.</li>
<li>Willingness to work evenings and weekends at all OHI sites as needed.</li>
<li>Ability to establish and maintain effective working relationships with other nurse practitioners, physicians, dentists and health care staff members.</li>
<li>Ability to work in a variety of settings such as the traditional office, mobile medical unit, etc.</li>
<li>Ability to effectively communicate orally with diverse individuals and groups.</li>
<li>Ability to work with patients of different cultures.</li>
<li>Ability to work within a team setting and to relate to the special needs of the patient population.</li>
<li>Provides consultation, guidance and direction to clinical support staff to ensure continuity and quality of services and care.</li>
<li>Active participant in continuing and maintaining a high level of professional skills and knowledge.</li>
<li>Works to ensure patient satisfaction by communicating with patients in a caring and compassionate manner.</li>
<li>Presents information to patients at an educational level consistent with their understanding.</li>

<li>Keeps all matters related to the organization confidential in compliance with confidentiality policy.</li>
<li>Participates in all safety programs which may include assignment to an Emergency Response Team.</li>

Education/Experience/Licensure
<ul>
<li>Master degree in Nursing from an accredited School of Nursing is required.</li>
<li>Three to five years of experience at RN level in a health care setting is required. </li>
<li>Active state of New Jersey RN and advanced practice nurse license are required.</li>
<li>Active state of New Jersey CDS and DEA license to prescribe and maintain controlled substances are required.</li>
<li>Certification by the New Jersey state board of nursing as an advanced practice nurse are required. </li>
<li>BLS for providers through the American Heart Association is required. </li>
<li>Proficiency in Microsoft Office 365 is required. </li>
