Our client is seeking a Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide psychiatric assessment, diagnosis, treatment, and medication management to patients in an outpatient, private practice setting. The provider will work with a multidisciplinary team (therapists, case managers, medical staff) to deliver trauma-informed, evidence-based, patient-centered care, helping clients manage mental health conditions across the lifespan (children, adolescents, adults). The PMHNP may also provide brief psychotherapy or counseling as needed/desired.

Key Responsibilities / Duties
· Conduct psychiatric evaluations, including history, mental status exam, risk assessment (suicidality, violence, self-harm), medical/psychiatric review, psychosocial factors.
· Diagnose mental health / psychiatric disorders using DSM-V-TR criteria.
· Develop and implement individualized treatment plans.
· Prescribe, adjust, monitor, and manage psychotropic medications and other interventions.
· Monitor for medication side effects, interactions, adherence, labs, and perform appropriate follow-up.
· Order, interpret, and follow up on laboratory tests, EKGs, imaging (as needed).
· Provide psychoeducation to patients and families about diagnoses, treatments, medications, side effects, behavioral management.
· Offer brief psychotherapy or counseling (e.g. supportive therapy, CBT elements, motivational interviewing) as appropriate.
· Collaborate with therapists, psychologists, social workers, primary care providers, and other professionals to coordinate care.
· Maintain accurate, timely, and compliant medical records in line with legal, regulatory, and clinical policies.
· Participate in case review, chart review, quality assurance, performance improvement initiatives.
· Engage in peer consultation and supervision (or mentorship).
· Possibly supervise or provide training to other nursing or clinical staff.
· Stay up to date with advances in psychiatric care, standard of care, clinical guidelines.

General Description:&nbsp;Carencia Psychiatric Nurse Practitioner will provide evidence-based therapeutic interventions with an emphasis on the patient’s health goals. A commitment to an outcomes-oriented approach and improvement in the quality of life is a central theme of services performed. Carencia takes a robust Mind, Body, Lifestyle approach to treatment which may also include Alternatives to Medicine (CAM), such as recommending supplements and other health recommendations.
 
Primary Purpose:&nbsp;To assist patients in mental health and substance abuse wellness and prevention through psychiatric nursing practice, including but not limited to; psychopharmacology, education, and psychotherapy.&nbsp;

Primary Responsibilities:&nbsp;
<ul>
<li>Screening – Conduct patient interviews and assessments</li>
<li>Orientation – Describe treatment experience to new patients</li>
<li>Assessment – Gather data from the patient and other sources, including administering standardized assessment scales; analyze and interpret the data to identify patient strengths, weaknesses, and needs.</li>
<li>Treatment Planning – Collaborate with the patient to develop treatment plans, identifying and prioritizing problems, goals, and treatment methods.</li>
<li>Counseling – Provide individual counseling, applying CBT and motivational interviewing skills.</li>
<li>Pharmacological- Utilize knowledge, safety, and best practices as it pertains to prescribing medications.</li>
<li>Education – Provide patients with information about substance abuse, co-occurring disorders, mental health, treatment, recovery, and resources.</li>
<li>Crisis management – Respond to patient needs during extreme distress or emergencies, during regular business hours.</li>
<li>Referral – Identify patient needs that can be met with other Carencia providers and/or outside of Carencia, and assist patient to use community resources and support systems.</li>
<li>Consultation – Relate with professionals on patient treatment and services to ensure comprehensive care.</li>
<li>Record Keeping – Maintain accurate, thorough, and up-to-date patient records.</li>
<li>Maintain client confidentiality and high ethical and professional standards.</li>
<li>Prepare for and participate in individual and group supervision and clinical staff meetings.</li>
<li>Assist the team in managing tasks and communications such as phone calls, emails, and messages.&nbsp;</li>
<li>Accurately collect patient financial responsibilities for services and bill insurance.&nbsp;</li>
<li>Conduct designated number of visits a week, in order to serve our patients best and meet their ongoing needs</li>
<li>Meet continuing education and other requirements to maintain professional licenses related to the position.</li>

Looks for feedback from patients and others working with Carencia to elevate each person’s experiences and always evaluates for opportunities for improvement. 
 
Physical Demands&nbsp;
 
While performing the duties of this job, the employee is required to stand, walk or sit; use a keyboard and computer mouse; reach with hands and arms; balance, stoop, kneel, bend talk and hear. The employee must occasionally lift and/or move up to 25 pounds.
