DUTIES AND RESPONSIBILITIES

<li>The Clinical Pharmacist will provide care for outpatients within their scope of training in collaboration with CCHS providers. Care will consist of assessment, ordering and interpretation of data, development and implementation of a therapeutic plan including prescription writing, and procedures within their capacity of expertise, protocols, and available equipment and appropriate follow-up. The Clinical Pharmacist will thoroughly, accurately, and promptly document all work completed in the practice management system</li>

<li>The Clinical Pharmacist will partner with the PCMH team (PCPs, Behavioral Health Consultant, Spiritual Health Provider and Nurses and support staff) at their assigned clinic to improve the health of the patients, identify high risk patients in the daily evening huddle that require detailed review by PCP or separate visit with the Clinical Pharmacist and help to maintain the overall cohesion of the clinic PCMH team.</li>
