RESPONSIBILITIES
General functions
Responsibilities specific to Discharge Planning include assessment, identification of specific needs, and social service intervention or referral. Upon individual recognition and staff referral, the discharge planner will contact the appropriate agency to meet the patient's social and emotional needs.
Partners and collaborates with the DON to ensure that policies and procedures of the organization are up to date, in line with adjustments in regulations from NYSDOMH, Federal, and other regulatory bodies, and are adequately communicated and reinforced with the nurses as a part of a more significant communication and training strategy. Collaborates with the DON to maintain policies and procedures for the effective implementation and tracking of monthly statistics for the nursing department. Works closely with the QI team to support maintaining program quality standards, benchmarks, and performance measures regarding nurse activities and outputs. Resolve conflicts and matters that could detriment the client and agency because of non-compliance of the nurse with established policies and procedures. Lead the investigation and resolution of client incidents and complaints. Collects data on common errors and omissions and present them at nursing meetings. Assists actively with the remediation of any issues with the data management system; can learn the documentation system and seek assistance from customer service to resolve any problems with the technical aspects of nursing documentation and input. Actively participates as a vital member of the agency Audit Team to ensure the correct submission of documents required during all in-person and remote and lead with remedying gaps and errors identified during the audit. About the position
Goodwin Home Health is currently seeking a full-time Clinical Nurse Manager that is able to work afterhours and weekend availability as needed for clinician support and is responsible for leading and supporting the nursing team to include RNs, LPNs, HHAs. The Clinical Nurse Manager will supervise the RNs, as well as support the case management of patients receiving skilled nursing care. This position will support the Compliance Officer by assisting with reviewing clinical information to assure all OASIS documentation is reviewed, accurate, and appropriate per CMS/ACHC guidelines and for billing purposes. The Clinical Nurse manager will assist with nurse scheduling, supporting field nursing with obtaining necessary orders and labs from MD offices, and collaborate with the Clinical Therapy Manager to assure the interdisciplinary team is working towards patient centered care and goals to support the mission of Home Health at Goodwin Living. Job Duties

<li>Performs/completes OASIS documentation</li>
<li>Performs a detailed assessment resulting in the development of a patient specific Plan of Care. Client clinical record documentation is done per agency policy</li>
<li>Provides direct nursing care in a Home Health setting under the guidelines of a physician’s order</li>
<li>Communicates with other team members regarding the Plan of Care and works closely with team members for optimal outcomes</li>

Qualifications:
• Registered Nurse with an active, current, and unrestricted license (required).

• Strong knowledge of Medicare/Medicaid guidelines, PDGM coding and process, OASIS accuracy/coding, and CMS/ACHC guidelines and requirements.
• Proficient analytical, negotiation, communication, problem solving and decision-making skills (required).

This may include patient care assessment, patient care planning and implementation, and working on patient care quality improvement. This position works in collaboration with the Program Director and the Medical Director to meet the Center’s program objectives and help ensure optimal healing outcomes and patient satisfaction. All clinicians working in a Healogics Center must perform their job responsibilities according to all Healogics policies, Hospital policies, as well as to accrediting organizations, federal and state regulation, and to the Centers for Medicare and Medicaid Services (CMS) guidelines, as applicable. Essential Functions/Responsibilities 
<ul>
<li>Proficient in all responsibilities related to Case Management. May be required to provide case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management. </li>
<li>Organizes and prioritizes clinical responsibilities, provides clinical oversight to clinical staff, such as Registered Nurses (RNs); Licensed Practical Nurses (LPNs) or Licensed Vocational Nurses (LVNs); Certified Medical Assistants (CMAs); Hyperbaric Oxygen Technicians, etc. </li>
<li>Collaborates with other health care providers, key hospital personnel, Wound Care Center Providers, Program Director, and Medical Director regarding clinical and patient needs. </li>
<li>Consistently looks at current processes and procedures, and identifies and implements areas of improvement, while complying with Health Insurance Portability and Accountability Act (HPAA), the Department of Health (DOH), as well as hospital and Healogics requirements</li>
<li>Engages in staff development, providing general in services as well as individual instruction and orientation. </li>
<li>Provides staff recognition through positive reinforcement and constructive feedback. </li>
