We are seeking a professional, self-motivated Clinical Assistant (LPN or CMA) to join our team that delivers primary care services to patients who reside in group homes. A Clinical Assistant is an essential care team member in our unique care model. Working alongside our healthcare providers, a Clinical Assistant’s duties include pre-visit planning and chart preparation, conducting patient screenings, reconciling medications, maintaining accurate patient charts, and performing minor procedures under provider supervision.&nbsp;
The Clinical Assistant provides collaborative nursing support working alongside a provider team traveling to group homes. The ADAPT Program is a best-in-class primary care delivery model based on clinical best practices for patients with Intellectual and Developmental disabilities and our teams have a deep knowledge of the unique Group Home setting of care.&nbsp;
Schedule: Full time position, day shift hours, no evenings, weekends or holidays. Hours are 8am to 5:00pm Monday thru Thursday &amp; 8am to 3pm on Fridays.&nbsp; Role is approximately 80% administrative/work from home (remote), and 20% in the field patient facing
Location: St. Paul, MN &amp; Surrounding Areas
Salary: $23.00 - $27.00 per hour. Salary will be commensurate with experience. 
Responsibilities:
<ul>
<li>Foster positive relationships with patients, families, and community staff members.</li>
<li>Pre-visit planning such as chart prepping, reviewing patient records, and following up on previous orders since the last visit.&nbsp;</li>
<li>Perform screenings for depression and cognitive function as part of Annual Wellness Visits.&nbsp;</li>
<li>Ensure medical records are updated accurately during and between visits.</li>
<li>Medication reconciliation.</li>
<li>Setting up e-prescriptions and orders under provider supervision.</li>
<li>Performing minor procedures under provider supervision, such as ear lavages, swabbing, and immunizations.</li>
<li>Monitor and respond to portal messages.</li>
</ul>
Qualifications:
Education/Certification/Experience
<ul>
<li>Active Licensed Practical Nurse (LPN) required or</li>
<li>Active Medical Assistant certification (CMA, RMA, or CCMA)</li>
<li>1-2 year’s experience working in a clinical setting preferred</li>
<li>Valid driver's license required</li>
</ul>
Knowledge/Skills/Abilities

<li>Knowledge of medical terminology and understanding of patient care notes</li>
<li>Ability to assist providers and facilitate an efficient, organized clinical flow</li>
<li>Ability to problem solve and work well under pressure</li>

Responsibilities:

<li>Responsible for the operation and management of the community in accordance with the standards and guidelines of the Owners/Board of Directors, Discovery Management Group, and federal/state/local laws and regulations.</li>
<li>Communicates and models a clear, customer focused vision, based upon a resident centered model of care.</li>

<li>Reviews monthly financial statements, implements plans of action for deficiencies.</li>
<li>Manages key, non-labor operating costs in line with budgeted levels.</li>
<li>Processes and submits monthly expenses and budget data timely per policies and internal business controls.</li>
<li>Meets NOI and occupancy expectations.</li>
<li>Oversees all departments, maintaining full responsibility for efficient operations and compliance with the financial goals established in the approved Operating Budget.</li>

Qualifications:
<ul>
<li>Bachelor’s degree, preferably in Business Administration, Hospitality, Hospital Administration, or Health related field.</li>
<li>Minimum of three years of managerial experience preferably in the senior living, health care or hospitality industries.</li>
<li>Previous management experience including hiring, coaching, performance management, daily operations supervision and discipline.</li>
<li>Previous sales experience preferred.</li>
<li>Demonstration of success in managing operating expenses.</li>
<li>Administrator License/certification</li>
<li>Provides leadership by exhibiting confidence in self and others; inspires and motivates others to perform well.</li>
