Center for Life Management (CLM) provides the area's most comprehensive array of mental health services. Our Integrated Care Program is an innovative approach to whole healthcare that encompasses the integration of treatment modalities for mental and physical health needs. Working together with a multi-disciplinary team, the Wellness Coach, a cross-trained mental health counselor and certified personal trainer, provides outpatient community-based support to assist client’s with improving symptoms, behaviors, and co-morbid disabilities. This is a 40 hours per week position.
 
Major Functional Areas and Description of Duties
<ul>
<li>Provide Functional Support Services (FSS) in the home and community-based settings as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan.</li>
<li>Engage client in the process of working toward and reaching their personal health, wellness and recovery goals by using motivational strategies and interventions.</li>
<li>Engage client in applicable assessments to monitor symptoms and social determinants of health.</li>
<li>Provide Crisis Intervention (CI) services, as needed, when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community.</li>
<li>Work closely with treatment team, integrated care programs (ex. Healthy Together), natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care.</li>
<li>Support Wellness Coach Team Leader in cultivating partnerships within the community to identify supplemental resources to offset program costs.</li>
<li>Provide timely documentation on each service delivered in accordance with agency policy.</li>

LPN Team Leader responsibilities will include:
<ul>
<li>Performs nursing skills and documents within the framework of licensure and as outlined on individual care plans; documents care delivery, observations, medications/treatments promptly and accurately.</li>
<li>Supervises and assists nursing assistants to follow their respective job descriptions. Confer with the Nurse Manager/Supervisor as indicated.</li>
<li>Responds rapidly and appropriately to emergencies involving residents and staff. This includes independent living resident calls, evacuations, falls, fire alarms and drills, or any other emergency situation.</li>

General Nature of Work
<ul>
<li> This is skilled work in a county jail. An employee in this class is responsible for the medical care of inmates. The Registered Nurse routinely and regularly has face to face contact with inmates. Registered Nurse must hold and maintain a Kentucky State Nursing License. Registered Nurses respond to medical emergencies and deliver an appropriate level of medical care to maintain the health and safety of the inmate. Registered Nurses may be exposed to infectious diseases and blood borne pathogens as a result of working with the inmate population. </li>
</ul>
Examples of Duties
<ul>
<li>Screen and assess inmates upon intake to ensure any medical concerns are properly treated and documented. </li>
<li> Evaluate and address inmate non-emergency medical complaints. </li>
<li>Respond to emergency medical incidents and ensure proper treatment is given to inmates as needed. </li>
<li>Medication management to include the transcription of providers orders, medication procurement, medication administration and the documentation of medication adherence. Medication management to include maintenance of medication inventory and handling all medication according to state and federal laws and regulations. </li>
<li>Work with other medical staff to ensure the cleanliness of the medical offices and exam room. </li>
<li>Make all appropriate medical documentations in the jail computer system and the inmates' medical file. </li>
<li>Communicate with other medical staff, the clinical director, and deputy jailers to ensure all inmate medical concerns are being addressed and all Clinical Director's orders are being followed. </li>
<li>Schedule appointments for inmates to go to outside providers when ordered by the Clinical Director, and notify the shift supervisor for appropriate transportation. </li>
<li>Follow all established treatment plans and monitor patients' progress. </li>
<li>Provide patient education at all clinical encounters and document in the medical record. </li>
<li>Document all inmate encounters in the medical record accurately and in a timely manner. Utilize and document nursing diagnosis in patient care. </li>
<li>Ability to work independently and without direct supervision. </li>
<li>Use universal precautions in all areas of practice. </li>
<li>Follow Boone County Jail's manuals to ensure the safety and security of the facility, inmates, and staff. </li>
<li>Generate sick call lists. </li>
<li>Maintain working inventory of all medical supplies. </li>
<li>Follow all PREA laws. </li>
<li>Must have a clear understanding of their job responsibilities and always practice according to the scope of practice of a Registered Nurse as described in the KY Nurse Practice Act. </li>
<li>Must wear proper medical attire. </li>
<li>Attend all mandatory staff meetings scheduled by the Health Service Administrator. </li>
<li>Maintain confidentiality of inmate health issues and appointments to ensure compliance with HIPPAA and maintain safety and security of the facility. </li>
<li>Perform history and physicals on all Immigration detainees. </li>
</ul>

<li>Must maintain a level of physical fitness that will allow the employee to perform duties satisfactorily. </li>

<li>Possession of or ability to obtain promptly, a Commonwealth of Kentucky motor vehicle operator's license for type of vehicle operated. </li>
