Summary/Objective: 
The Foster Care Case Planner uses a diverse and strength-based approach for the direct interaction with the child, the Foster Parents, the child's birth family, and the community. The Case Planner acts as a liaison with various private as well as public service providers and resource agencies to coordinate all case management needs of the youth and family. The Case Planner prepares placement &amp; discharge summaries, writes treatment &amp; crisis management plans, attends court proceedings, assists the agency in the implementation of independent living services, and participates in the crisis response on-call rotation. 
Essential Functions:
<ol>
<li>Completes initial Family Assessments on each parent, in addition to each tracked child.</li>
<li>Maintains contact with all service providers in regard to all family participants attendance and progress toward achieving treatment goals as they relate to required services.</li>
<li>Coordinates visitation for each youth with an identified resource. Provides supervision for visitation regularly to assess the family’s progress.</li>
<li>Meets and/or maintains contact with each youth and family member in accordance with the initial Level Agreement and LDSS Contract.</li>
<li>Works as a member of a Treatment Team, including the child and family, to develop an Individualized Treatment Plan and assist with identifying goals for the family and each member of the family.</li>
<li>Provides Supportive Counseling to the child, birth parents and foster parents. </li>
<li>Participates in regularly scheduled Team Meetings.</li>
<li>Work with community agencies, organizations, and systems for long-term planning and support of the families and children served.</li>
<li>Attend all Family Court hearings, aide in the preparation of necessary court documentation, and provides testimony.</li>

Putnam Court is a building which was developed and is operated by Dunn Development Corp., a private developer.&nbsp; Putnam Court is a newly constructed residential building located on Putnam Avenue between Irving Place and Downing Street in the Clinton Hill neighborhood of Brooklyn.&nbsp; The project consists of a seven-story elevator building containing 59 residential units, with 58 units of rental housing and one superintendent’s unit. CAMBA, Inc. is the on-site services provider at Putnam Court. Thirty-four apartments, 58% of the units in the building, will be master leased by CAMBA and will house Population A chronically homeless single adults who are diagnosed with SPMI (seriously and persistently mentally ill) or MICA (mentally III and chemically addicted). The remaining 24 units are occupied by community residents, who may require some services.

What The Program Manager Does:

<li>Communicate with peers in other CAMBA programs on issues related to client progress and best program practices.</li>

CNBC is seeking an enthusiastic Associate Producer to join our growing digital video team. The Associate Producer will be a talented reporter and creative editor, has a passion for visual storytelling and business news, and enjoys in depth reporting on issues pertaining to markets and politics. He or she should have a working knowledge of the American political system and marketplace. Benefits of employment with the Superior Court, County of Madera include 13 paid holidays and 1 floating holiday per year, 1 sick leave day per month (unlimited accrual); Appointed to a permanent position shall be granted 5 days credit for vacation following the completion of 6 calendar months of continuous paid full-time service and following the completion of 12 calendar months of continuous paid full-time service in a permanent position will be credited 5 additional vacation days, vacation accrual increasing thereafter with years of service (50 days max accrual); Membership in the California Public Employees Retirement System (CalPERS) pursuant to the provisions of Californias 2013 Pension Reform Act (Act) and CalPERS Regulations. Program Summary
Rising Strong serves families who are at risk for separation due to parental substance misuse by providing safe housing, substance misuse treatment, case management, and other supportive wraparound services. By serving whole families, our aim is to reduce trauma in children, decrease the rates of foster care placements, and remove barriers that traditionally prevent parents from succeeding in substance abuse treatment. We walk alongside and support vulnerable families as they seek to change their stories.
 
Job Summary
This position models appropriate health and safety standards for all individuals and familiesand helps to support additional health &amp; sanitation tasks during overnight hours. This position coaches, practices, encourages, and empowers families in reunification and recovery both short and long term, with a focus on providing support during overnight hours for families residing at Rising Strong.
Job Duties/Responsibilities
To perform this job successfully, an individual must be able to satisfactorily perform each of the essential duties/responsibilities listed below. Reasonable accommodation may be made to enable individuals with disabilities to perform the essential duties/responsibilities of this job.
Spend 40% of time supporting healthy shelter operations during overnight hours, which includes:
<ol>
<li>Complete assigned list of health &amp; sanitation tasks each night/throughout the building.</li>
<li>Report to RS Leadership when supplies run low/or when additional supplies are needed.</li>
</ol>
Spend 60% of time supporting the additional Family Skills coach duties, listed below:
<ol>
<li>Assist residents, as needed, with urgent care issues.
<ul>
<li>Support families in after-hours training for any urgent health care needs that arise.</li>
</ul>
</li>
<li>Monitor the building for safety and potential risks to sobriety.
<ul>
<li>Conduct visualbed checksin partnership with another FSCtwo times a night to ensure that families are not co-sleeping &amp; to ensure that all dependents are in their assigned rooms each night.</li>
<li>Conduct walk-throughs of common spaces throughout the building to ensure safety is maintained throughout the nighttime hours.</li>
</ul>
</li>
<li>Maintain family records, Shift Notes and complete all other necessary documentation.
<ul>
<li>Document bed checks and other notable events in Shift Notes each night.</li>
<li>Document the head count/number of families that are currently residing in the building in Shift Notes each night.</li>
</ul>
</li>
<li>Conduct urinalysis testing, as requested by staff leadership.</li>
<li>Assist residents, as needed, with solving problems and conflict resolution. The goal is creating and identifying processes and routines that can be carried out successfully to life outside the program.</li>
<li>Coach families on policies and procedures to ensure that Rising Strong rules are being followed and that families are practicing safe habits.</li>
<li>Carry out shift responsibilities to ensure the ongoing safety, cleanliness, and smooth operation of the facility.
<ul>
<li>Train and implement Emergency Evacuation protocols according to Rising Strong Evacuation Procedures for both scheduled &amp; un-scheduled events.</li>
</ul>
</li>
<li>Intervene in emotionally charged situations to deescalate conflict.</li>
<li>Pay attention and intervene when families are isolating from the community.</li>
<li>Maintain a high alert for safety threats for all family members and intervene immediately to remove or reduce the threat.</li>
<li>Ensure no families leave the facility without authorization from appropriate staff members.</li>
<li>Report any suspicion of drug or alcohol use to the appropriate staff members immediately.</li>
<li>Assist in calmly and compassionately responding to relapses with staff leadership, with safety for all parties as the priority.</li>
<li>Maintain hygienic and organized supply areas.</li>
<li>Assist with leading activities for parents and children.</li>
<li>Assist with transporting families as assigned.</li>
<li>Answer the phone and take messages.</li>
<li>Provide leadership and appropriate response in case of an emergency.</li>
<li>When subpoenaed or otherwise called upon, appear in court to testify regarding families involvement in the program.</li>

Job Qualifications

Education/Experience:to perform this job successfully, an individual must have an associate degree or an equivalent combination of education and experience. Experience working with children required. At least two years experience in social services preferred; working with individuals in recovery, the child welfare system, homelessness, residential environments, and/or mental health strongly preferred.
Certificates/Licenses:To perform this job successfully, an individual must:

<li>Valid Washington State Drivers License</li>
