· Act as a patient advocate and liaison between the patient/family and GROW screening/ Roc Family Teleconnects
· Record patient care management information in the EMR (training provided) and other
software no later than 24 hours after patient contact
· Coordinate transportation an or other services to assist patient in availing themselves to health care needs
· Works in conjunction with Care managers and QI team on patient gaps in care (vaccinations)
· Assists in training and education for new Community Health Workers and current Community Health Workers
· Uses computer applications such as spreadsheets, word processing calendar, email and database software in performing work assignments

Job DescriptionDescription:Title 1 is a federally funded program that provides extra academic support to students in the areas of reading and/or math. The purpose of the title is to ensure that all children have an equal and significant opportunity to obtain a high-quality education. LMCS is looking for a lead title teacher with a dual role of librarian. Montessori training is available and encouraged. Requirements:

Candidates must hold, or be eligible to hold, a certificate issued by the Wyoming Professional Teaching Standards Board with an endorsement as an Elementary Level (K-6) teacher. 
 
QUALIFICATIONS:

<li>Administers subject specific assessments for the purpose of assessing student competency levels and/or developing individual learning plans. </li>
<li>Advises parents and/or legal guardians of student progress for the purpose of communicating expectations; student's achievements; developing methods for improvement and/or reinforcing classroom goals in the home environment. </li>
<li>Assesses student progress towards objectives, expectations, and/or goals for the purpose of providing feedback to students, parents and administration. </li>
<li>Collaborates with instructional staff, other school personnel, parents and a variety of community resources for the purpose of improving the overall quality of student outcomes, achieving established classroom objectives in support of the school improvement plan. </li>

<li>Monitors students in a variety of educational environments (e.g. classroom, playground, field trips, etc.) for the purpose of providing a safe and positive learning environment. </li>
<li>Participates in a variety of meetings, workshops, training and seminars for the purpose of conveying and/or gathering information required to perform functions. </li>
<li>Prepares a variety of written materials (e.g. grades, attendance, anecdotal records, etc.) for the purpose of documenting student progress and meeting mandated requirements. </li>
