Supervision and Direction: The Maintenance Person is responsible for operating and maintaining electrical and mechanical equipment used to provide heating and hot water for the property, assume the provision and maintenance of efficient building heat and air-conditioning functions including safety, maintenance, repairs, and other related services. The Maintenance Person will receive direction and instruction from the Site Manager and will coordinate their daily activities with that person. 
Requirements:
Duties and Responsibilities: • Maintain heaters, air-conditioners, and other equipment in the buildings; to insure that the equipment is operating safely and efficiently; ensure continuous hot water service; determine by inspection that all control equipment is operating properly; check such items as circuit breakers, switches, relays, starters, connections and filters for mechanical and electrical faults, sign of wear, dirt, overloading, and other indications of trouble; and perform such preventive maintenance as lubricating motor hearings and pumps, replacing brushes and filters. • Perform all necessary maintenance and repairs on apartments. • Perform all necessary work to turn over units as they are vacated, including furniture removal • Immediately report any health or safety issues to Site Manager • Weekly inspect buildings, grounds and other public areas making a report on an • inspection form and consult with the Site Manager regarding deficiencies. • Periodically inspect building and grounds to determine necessary preventive maintenance needed and consult with the Site Manager regarding such conditions. • Provide adequate and efficient housekeeping functions that assure cleanliness of all building halls, walls, stairways, grounds and other public areas, including cleaning light fixtures and mailbox fronts. • Clean washers and dryers in laundry rooms on a regular basis. • Possess a valid driver’s license and a driving record acceptable to TMAM for site errands, including trips to banks, stores and other vendors. • Pick up trash each day, clean sidewalks, edge all walks, cut and edge all grass around all buildings, trees, plants, etc. • Report building damage or vandalism, unusual circumstances and conditions, and other related incidents to the Site Manager. • Perform physical duties required to correct emergency conditions such as water flooding, sewer back-ups and responding to inclement weather events, including shoveling and chemical distribution • Ensure that all maintenance work and repairs are properly recorded on work orders
• Must be able to lift a minimum of 50 pounds
• Delivery of appliances to units due to transfers, removals and/or replacements
• Able to climb ladders to a height of 20 feet
• Cut grass, trimming and other landscaping duties to maintain outside property areas in an acceptable manner using power equipment such as but not limited to mowers and snow blowers

Supervisory Responsibilities: LPNs, Nurses, Nurse Coordinators
Typical Schedule: Primarily M-F Days. Must be willing and able to work flexible hours/days, (can work a hybrid schedule –remote and in-person) but is required to be 4 days per week in person/on-site, with supervisor approval. Responsible for emergency call rotation/on-call for one week every 6 weeks. 
Position Summary
The Health Services Manager oversees the Health Services Department to ensure quality medical practices and care for the individuals served by Hammer &amp; NER. The Health Services Manager provides direct supervision to the nursing team in addition to completing regular nursing duties. They work to support the nursing team function optimally and will step into complete assessments of health status and provision of direct nursing care when needed. All employees are expected to center, model, and champion Hammer’s core values: Person-Centered, Relational, Opportunistic and Stewardship.
Primary Duties and Responsibilities

</ol>

<li>Provide nursing and health care support to people served within the scope of nursing practice. </li>
<li>Responsible for a small caseload of supported individuals at times and support the nursing team with caseload coverage</li>
<li>Ensure that nursing standards are maintained and that all applicable rules and regulations are followed.</li>
<li>Work as part of the inter-disciplinary team to ensure best practice for the individuals we support.</li>
<li>Provide clinical supervision and consultation as needed, including for hospital and TCU stays, and hospice care.</li>

<li>Provide direction and supervision to nursing staff, including the provision of timely and effective feedback related to organizational policies and procedures and medical regulations.</li>
<li>Provide guidance in absence of Senior Director of Program Services.</li>
<li>Complete weekly check-ins and ongoing communication with direct reports.</li>
<li>Actively participate in team building, fostering a work environment that reflects the organizations stated core values.</li>
<li>Participate in the hiring of staff members to ensure quality work reflective of Hammer’s mission, vision and values.</li>

<li>Assists with special projects and completes other duties as assigned by the Senior Director of Program Services </li>
<li>Act as liaison with outside vendors including developing and maintaining contracts, as well as working with outside agencies to improve services in our field.</li>
<li>Develop and facilitate trainings for the direct care workforce to help meet the needs of the people we support. </li>
<li>Act as Infection Control Officer for the organization and consult with a physician regarding these issues.</li>
<li>Participate in the Director’s team meetings as requested.</li>
<li>Participate in the development of nursing and medical projects, procedures and policies, and the communication and implementation of these projects, </li>
<li>Frequently organizes multiple responsibilities at once, requiring time management, organizational and reporting skills, and proven ability to achieve assigned deadlines.</li>

Essential Knowledge and Qualifications
<ul>
<li>Licensed as a Registered Nurse in good standing with the State of Minnesota. </li>
<li>Bachelor of Science in Nursing (BSN) preferred. </li>
<li>Experience and competence in managing health services for people who have disabilities and other related conditions. </li>
<li>Experience supervising, coaching, and mentoring a team of employees to provide effective health services.</li>
<li>Experience working with the aging population, particularly aging in the individuals with disabilities</li>
<li>Demonstrated thorough knowledge and understanding of current health related practices, issues, and regulations relative to people with disabilities and related conditions.</li>
<li>Demonstrate the ability to effectively teach, train and mentor staff to provide effective healthcare services to people.</li>
<li>Excellent written and verbal communication skills including fluency in English and proficiency with Microsoft Office (especially Outlook and Excel) and SharePoint. </li>
<li>Available to work a flexible full-time schedule including some evenings, weekends, and overnights.</li>
<li>Requires a valid driver's license, personal auto and driving record that meets Hammer &amp; NER’s insurance requirements. </li>
<li>Must receive a MN DHS Background study clearance and show freedom from Tuberculosis. </li>

<li>Ability to bend and move to perform nursing head to toe assessments, evaluations, lifting, and are able to complete other skilled nursing tasks.</li>
<li> Ability to lift 40lbs (such as lifting a wheelchair into a van), pushing, or pulling an individual in a wheelchair or while assisting with personal cares.</li>
<li>Physical cares may include twisting/turning, kneeling/squatting, reaching, and assisting with pivot transfers. </li>
<li>Operating specialized medical or adaptive equipment, including use of lifting equipment.</li>
<li>Must demonstrate the ability to work diplomatically with a wide variety of people, including people served, families, legal representatives, and other medical professionals. </li>
