<li>You will work with Individuals and their “circle of support” (social agencies, health care providers, etc.) and family members to ensure that our behaviorally challenged and/or medically fragile Individuals can develop and/or maintain their own homes and daily routines. You do this by providing first-hand training and developmental support across a broad spectrum of activities, e.g., daily living skills, mobility training, developing coping skills, navigating the community, money management, safe, self-medication management and other customized interventions based on the Individual’s Service Plan. </li>
<li>You will also take care to document and complete all reporting in an accurate and timely manner and engage in proactive and open communications with all key stakeholders, including recommendations for modifications, alternative training methods or need for adaptive equipment, etc. to ensure the personal care and safety of your assigned Individual(s).</li>
</ul>

<li>You will participate in all required training, team, and company meetings to build your knowledge, skills, and abilities to grow in the family. We like to promote from within our own house!</li>

Responsibilities
Elwyn is currently seeking a Mental Health Professional to support our wonderful new Young Adult RTF (Residential Treatment) program, set to launch in March 2025.
 The Mental Health Professional will develop, implement, and document individual progress for individuals with behavioral health and forensic issues who are residents of Elwyn's Young Adult Residential Treatment Facility (YA-RTF). Provides supervision and programmatic decision-making. Completes mental status exams utilizing current knowledge of the Diagnostic and Statistical Manual of Mental Disorders, version IV (DSM-IV). Provides supportive counseling and acts as an advocate to represent the individual's needs and rights with all outside support service agencies, families, etc. Assists and supports individuals in all activities of daily living. Designs and facilitates therapeutic groups. Plans and implements community integration plans.
DUTIES AND RESPONSIBILITIES:
<ul>
<li>Develop and implement individual programs for individuals with behavioral health challenges and provide other individualized case management functions as directed by supervisor or director </li>
<li>Provide supervision and program implementation </li>
<li>Provide all appropriate documentation set forth by director and regulations including monthly summaries and individual behavioral/psychiatric assessments </li>
<li>Provide mental status exam using the current DSM </li>
<li>Provide supportive counseling to individuals with personal, behavioral, or social challenges </li>
<li>Develop and ensure individual's adherence to behavior management plans </li>
<li>Advocate for individuals with outside support agencies </li>
<li>Interface with families and other interested parties within the limits of signed consents </li>
<li>Design and facilitate therapeutic groups according to individual need </li>
<li>Plan and implement community integration plans for individuals </li>
<li>Maintain communication with medical physician, psychiatrist, and pharmacist as needed </li>
<li>Maintain communication with appropriate ancillary service providers </li>
<li>Assist and train consumers in all aspects of self- care to ensure a maximum level of independence for that individual. These areas will include all aspects of personal hygiene, shopping, cooking, meal serving, budgeting, laundry, and housekeeping </li>
<li>Ensure a safe and a clean-living environment for individuals </li>
<li>Intervene in crisis situations in accordance with established agency protocol </li>
<li>Encourage individuals in the constructive use of leisure time </li>
<li>Escort individuals to and from appointments </li>
<li>Attend scheduled staff meetings </li>
<li>Facilitate resident meetings</li>
<li>Attend appropriate in-service as directed by supervisor and required by funder and regulatory bodies </li>
<li>Participate as a member of the interdisciplinary team </li>
<li>Conduct fire drills and building inspections and provide the appropriate documentation for same </li>
<li>Report all problems or concerns to supervisor or director </li>
<li>Remain on duty until relieved </li>
<li>Maintain current certification/recertification of CPR, First Aid, and Crisis Intervention. Responsible for adherence to standards, procedures and schedules established by the program administration </li>
<li>Perform cardiopulmonary resuscitation (CPR)and crisis intervention using agency-trained protocols. </li>

 QUALIFICATIONS, EDUCATION AND EXPERIENCE:
<ul>
<li>Bachelor's degree in a human service or related field, including but not limited to medicine, social work, psychology, nursing, occupational therapy, recreational therapy, or vocational rehabilitation with one (1) year experience in adult behavioral health services OR Bachelor's degree other than in human services or a related field and two (2) years' experience in adult behavioral health services </li>
<li>Demonstrated ability to work effectively as part of a team </li>
<li>Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities </li>
<li>Demonstrated strong attention to detail </li>
<li>Demonstrated strong time management and organizational skills </li>

Mental Health Professional - 2nd shift

Elwyn is currently seeking a Mental Health Professional to support our wonderful new Young Adult RTF (Residential Treatment) program, set to launch in March 2025. The Mental Health Professional will develop, implement, and document individual progress for individuals with behavioral health and forensic issues who are residents of Elwyn's Young Adult Residential Treatment Facility (YA-RTF). Provides supervision and programmatic decision-making. Completes mental status exams utilizing current knowledge of the Diagnostic and Statistical Manual of Mental Disorders, version IV (DSM-IV). Provides supportive counseling and acts as an advocate to represent the individual's needs and rights with all outside support service agencies, families, etc. Assists and supports individuals in all activities of daily living. Designs and facilitates therapeutic groups. Plans and implements community integration plans.
 $19.00 per hour Eligible for $2000 sign on bonus Comprehensive Benefits 2nd shift (3 pm - 11 pm), four weekdays and every other weekend 
DUTIES AND RESPONSIBILITIES:
<ul>
<li>Develop and implement individual programs for individuals with behavioral health challenges and provide other individualized case management functions as directed by supervisor or director </li>
<li>Provide supervision and program implementation </li>
<li>Provide all appropriate documentation set forth by director and regulations including monthly summaries and individual behavioral/psychiatric assessments </li>
<li>Provide mental status exam using the current DSM </li>
<li>Provide supportive counseling to individuals with personal, behavioral, or social challenges </li>
<li>Develop and ensure individual's adherence to behavior management plans </li>
<li>Advocate for individuals with outside support agencies </li>

General Summary
Under limited supervision, the Forensic Nurse Examiner (FNE) provides comprehensive care to sexual assault patients. The FNE conducts a medical forensic assault history and physical examination as well as evidence collection, preservation, and documentation. The FNE ensures crisis intervention and medical treatment is provided while in the Emergency Department. The FNE coordinates reporting to appropriate police jurisdiction and/or Child Protective Services, if applicable. The FNE will interface with patient advocacy, law enforcement and crime labs. The FNE provides referrals to community agencies as well as follow-up referrals for health care. The FNE will participate in legal proceedings including courtroom testimony.
The FNE provides care to patients via therapeutic use of self, the nursing process, the environment/technology, and other healthcare team members, according to established standards. Patients served include all age groups (infant through older adult as identified in Performance Standards) dependant on FNE level of certification.

Job DescriptionFull- time- Non Exempt&nbsp; Hours: Monday- Friday 9am- 5pm (some night, weekend, holiday on-call availability) Wage Range: $32.87- $36.52&nbsp; Job Summary The Medical Examiner’s Office responds to reports of death 365 days a year (including evenings, overnight, weekends and holidays), The Lead Medicolegal Death Investigator (MDI) is used to fill vacancies when the Medical Examiner is not available, and when there are simultaneous calls for service. The Lead Medicolegal Death Investigator is the lead investigator under the Medical Examiner. The Lead MDI assumes Medical Examiner daily activities in the absence of the Medical Examiner. The Ozaukee County Medical Examiner’s Office operates under the statuary obligation of determining cause and manner of death, along with the obligation of identifying deceased individuals, and locating and notifying legal next of kin. The Lead Medicolegal Death Investigator determines the manner of death through an investigative process, using deductive and inductive reasoning skills to draw conclusions based on existing knowledge, prior experience and available evidence. In cases of sudden or unattended deaths such as those resulting from accidents, homicides, suicides, overdoses or other unusual circumstances, the Lead Medicolegal Death Investigator relies on the expertise of a forensic pathologist to medically determine the cause of death. <li>Examine the body for trace evidence.</li>
<li>Outlines the scope and course of a death investigation, determines whether to perform an autopsy, toxicology, or if other laboratory tests are needed as required by law.</li>
<li>Preserves and documents evidence.</li>
<li>Establish positive decedent identification through scientific methods if necessary.</li>
<li>Notifies the next of kin or assists with the search for next of kin when necessary.</li>
<li>Secures and prepares the body and personal belongings for transport to the morgue, ME’s office or release to a local funeral home.</li>
<li>Exercises the authority to determine the official cause and manner of death based on investigative findings, evidence, and consultation with the Medical Examiner and forensic pathologists as necessary, and has the authority to sign the death certificate.</li>
</ul>
 Additional follow-up is often required and noted in the section below. &nbsp;20% Investigation Follow Up (Phone Inquiries/Notifications/Coordination): Colleague Interactions:
<ul>
<li>Intakes data from deaths that occur at medical facilities, and determine Medical Examiner jurisdiction</li>
<li>Collects and reviews medical records and history, consulting with attending or primary care physicians to identify factors contributing to the death.</li>
<li>Communicates with Next of Kin throughout death investigation.</li>
<li>Coordinates transports to/from scene, to/from Morgue and to/from ME office.</li>
<li>Corresponds with Milwaukee ME Office.</li>
<li>Authorizes organ donation via Donor Network.</li>
<li>Supplies toxicology and autopsy reports to DA/law enforcement agencies.</li>
<li>Testifies in civil and criminal court cases when necessary.</li>
</ul>
 30% Cremation Requests and Permits
<ul>
<li>Obtaining Cremation Request Forms from Funeral Homes</li>
<li>Scheduling/coordinates views with Funeral Homes</li>
<li>Perform external body examination and photography of the body for every view</li>
<li>Input all information obtained in the electronic case management system</li>
<li>Reviews the signed death certificate to ensure there are no discrepancies or suspicion of unnatural causes of death. </li>
<li>Once all documentation is verified the Cremation Permit may be signed by the Lead MDI or Medical Examiner and issued to the requesting funeral home. </li>
</ul>
 10% Team Management and Supervision:
<ul>
<li>Supervises, coordinates, and directs the action of MDI as requested under the direction of the Medical Examiner</li>
<li>Reviews investigative reports</li>
<li>Direct educational and training programs for staff members and law enforcement agencies and others as requested under the direction of the Medical Examiner</li>
</ul>
 10% Morgue Operations and Other Duties:
<ul>
<li>Maintains morgue admission and releases to/from Morgue, other ME offices, and funeral homes</li>
<li>Maintain vehicle cleanliness and stocking of ME vans and cots</li>
<li>Assist the Medical Examiner in maintaining finances and logs at the discretion of the Medical Examiner</li>
</ul>
 Supervision Exercised Supervises Medicolegal Death Investigators in the absence of the Medical Examiner. Minimum Education Qualifications Education and/or Experience Requirements:
<ul>
<li>Graduation from a recognized college or university with a bachelor’s degree in such areas as biological or medical science, anatomy, anthropology, nursing, or closely related field AND 3 years of work experience in medicolegal death investigation&nbsp;OR&nbsp;5 years of work experience in medicolegal death investigation, as an autopsy assistant or closely related field.</li>
<li>Requires demonstrated knowledge of medical terminology, medicine as it pertains to death and varying forms of trauma.</li>
<li>Using medical and legal investigative techniques and procedures in a setting consistent with the duties of the position.</li>
</ul>
 Licenses, Certifications, and Other Requirements:
<ul>
<li>Must have a valid Wisconsin Driver’s License.</li>
<li>Must successfully pass criminal background check and preemployment physical.</li>
<li>Ability to receive ABMDI certification during employment. </li>
</ul>
 Candidates must meet these requirements or possess a combination of similar education, training, and experience that provides the knowledge, skills, and abilities necessary to perform the duties of this position. Minimum Knowledge, Skills, and Abilities Qualifications In order to perform the functions and responsibilities of the position (listed above) the following knowledge, skills, and abilities are essential. &nbsp;
<ul>
<li>1st Shift Position that requires some evening, nights, and weekend availability.</li>
<li>Alternating weekly on-call, as back up for the investigators. </li>
<li>Must commit to monthly on-call hours which may include daytime, evening, weekends, and holidays.</li>
<li>Valid Wisconsin driver’s license and access to personal transportation, or other reliable means of city and rural travel on a 24-hour basis.</li>
<li>Knowledge of the statutory duties affecting the Medical Examiner’s Office, methods of conducting death investigations, and supervisory techniques. </li>
<li>Ability to establish and maintain effective public relations and working relationships with local, state and federal law enforcement agencies. </li>
<li>Ability to exercise judgment and make independent decisions to oversee the work of staff, communicate effectively, orally, and in writing, and interact effectively with co-workers and the general public. </li>
</ul>
 In evaluating candidates for this position, The Medical Examiner’s Office may consider a combination of education, training, and experience which provides the necessary knowledge, skills, and abilities to perform the duties of this position.&nbsp; Physical Requirements This work requires the occasional exertion of up to 100 pounds of force; work regularly requires sitting, frequently requires standing, speaking or hearing, using hands to finger, handle or feel and repetitive motions and occasionally requires walking, stooping, kneeling, crouching or crawling and reaching with hands and arms. Work requires close vision, distance vision, ability to adjust focus, depth perception, color perception, night vision and peripheral vision. Work Environment &nbsp;
