GENERAL SUMMARY

Responsibilities include but are not limited to:

<li>Fill new medication orders and cassettes and prepare for Pharmacist's review.</li>

<li>Enter appropriate information-such as NDC, manufacturer, bar-code, lot #, expiration date-for each medication being packaged.</li>
<li>Performs duties as assigned by Pharmacy Manager, and Staff Pharmacist including utilizing pharmacy systems to enter patient and drug information, ensuring information is entered correctly, filling prescriptions by retrieving, counting and pouring pharmaceutical drugs, verifying medicine is correct, and checking for possible interactions. Assists pharmacists in scheduling and maintaining work flow.</li>
<li>Reports, immediately, prescription errors to pharmacist on duty and adheres to Company policies and procedures in relation to pharmacy errors and the Quality Improvement Program.</li>
<li>Prepare IV medications and parenteral nutrition (TPN) utilizing USP 797 and 800 guidelines for review.</li>
<li>Label pre-mixed IV meds and batch prep mini bag plus IV meds for pharmacist's review.</li>
<li>Performs other position related duties as assigned.</li>
<li>Employees shall adhere to high standards of ethical conduct and will comply with and assist in complying with all applicable laws and regulations. This will include and not be limited to following the Solaris Health Code of Conduct and all Solaris Health and Affiliated Practice policies and procedures; maintaining the confidentiality of patients' protected health information in compliance with the Health Insurance Portability and Accountability Act (HIPAA); immediately reporting any suspected concerns and/or violations to a supervisor and/or the Compliance Department; and the timely completion the Annual Compliance Training.</li>

Chronic Disease Management:

Additional Duties:

<li>Assess patient’s response to drug therapy and plan drug therapy based on physician-established diagnoses.</li>
<li>Order and evaluate laboratory tests necessary to evaluate drug therapy effects and outcomes.</li>
<li>Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states (e.g., hypertension, hyperlipidemia, anticoagulant, diabetes, asthma, refill clinics, etc.) in cooperation with the medical staff.</li>
<li>Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics (e.g., aminoglycosides, phenytoin, antithrombotic).</li>
<li>Administer prescription or non-prescription drugs according to established agreements or MTF protocols.</li>
<li>Assess metabolic needs and order therapeutic enteral or parenteral nutrition products in the inpatient setting.</li>
<li>Evaluate medical and medication histories for drug-related problems and adjust drug therapy accordingly.</li>
<li>Consult with other healthcare providers (e.g., physicians, dietitians, nurses, physical therapists, etc.) about patient treatment needs or options.</li>

<li>Consult with other specialty practitioners to provide optimal patient pharmaceutical care.</li>

 Basic Qualifications 
<ul>
<li>BS in Pharmacy or Pharm D Degree from an accredited educational institution.</li>
<li>Current pharmacist licensure in the states within the district.</li>
<li>Certified Immunizer or willing to become an immunizer within 90 days of hire.</li>
<li>At least 1 year pharmacy experience including prescription filling, recordkeeping, legal compliance, pharmacy operations, pharmacy software and technology systems and insurance billing. (Some states may require more specific pharmacy experience in which case those requirements would take precedent).</li>
<li>Experience performing prescription dispensing activities that demonstrate a strong working knowledge of applicable state and federal controlled substance laws.</li>
