The Clinical Nurse Care Manager provides ongoing care coordination to individuals with both physical and behavioral health conditions. Patients with at least two chronic conditions qualify for more intensive management to ensure understanding of the treatment plan and that their conditions are managed as expected. JOB DUTIES

As a Home Health Clinical Manager (RN), here’s a big-picture view of what you’ll do:

<li>Assure ongoing interdisciplinary assessment of the patient.</li>
<li>Ensure our home care clinicians meet and exceed standards for safe, effective and efficient clinical practice through support and oversight of our home care staff in accordance with all applicable federal and state laws, rules and regulations.</li>

Essential Functions/Responsibilities

<li>Proficient in all responsibilities related to Case Management. May be required to provide case management to a group of patients providing specific assessments according to protocols and procedures, clinical competence in procedural skills, treatments, and patient/caregiver education related to wound care management. </li>
<li>Organizes and prioritizes clinical responsibilities, provides clinical oversight to clinical staff, such as Registered Nurses (RNs); Licensed Practical Nurses (LPNs) or Licensed Vocational Nurses (LVNs); Certified Medical Assistants (CMAs); Hyperbaric Oxygen Technicians, etc. </li>
<li>Collaborates with other health care providers, key hospital personnel, Wound Care Center Providers, Program Director, and Medical Director regarding clinical and patient needs. </li>
<li>Consistently looks at current processes and procedures, and identifies and implements areas of improvement, while complying with Health Insurance Portability and Accountability Act (HPAA), the Department of Health (DOH), as well as hospital and Healogics requirements</li>
<li>Engages in staff development, providing general in services as well as individual instruction and orientation. </li>
<li>Provides staff recognition through positive reinforcement and constructive feedback. </li>
<li>Collaborates with Program Director in coordination of program objectives, from a budgetary and financial perspective</li>
<li>If the program also provides Vein Care services, coordinates vein clinic and assists in providing care as needed after Healogics Vein Clinic training completed</li>

Required Education, Experience And Credentials
