Role Scope: 
We are looking for a dynamic, service-oriented dietitian who is passionate about supporting elderly persons in their journey. The Dietitian will work within a multidisciplinary care team led by a primary care provider to ensure whole-person, integrated care. Neighborhood Healthcare PACE is a managed medical plan built around surrounding participants with a team of physicians, nurses, social workers, therapists and care coordinators to help them maintain good health and a good quality of life. Our goal is to keep our seniors happy and healthy at home surrounded by their family and community.
As a private, non-profit 501(C) (3) community health organization, we serve over 500k medical, dental, and behavioral health visits from more than 100,000 people annually. With two PACE centers located in Riverside County, our PACE program is positioned to serve over 650 senior participants.
The PACE Registered Dietician (RD) will design and implement nutrition education programs at Neighborhood Healthcare. This role will evaluate participant nutritional status, develop/implement nutritional care plans, and communicate nutritional information to providers and other health professionals. Additionally, this role will provide dietary counseling and education to patients. Schedule: M-F 8:00am - 4:30pm.
Responsibilities
<ul>
<li>Conducts individual nutrition assessments to establish nutrition care plans using medical data to determine health risks and needs for patients, including therapeutic diets</li>
<li>Provides diabetes education to patients, including risk factors, signs/symptoms, medical nutrition therapy, blood glucose monitoring, and medication management</li>
<li>Evaluate patient glucose logs and assess medication dosing adjustments, as needed</li>
<li>Supports individual, family, and group nutrition/chronic disease education services by creating culturally appropriate curriculums, planning programs, delivering education, and evaluating effectiveness</li>
<li>Serve as a resource for health education staff regarding American Dietetic Association Standards and CPSP and PACE guidelines</li>

Education/Experience
