The Nurse Practitioner/ Physician Assistant provides high quality, patient-focused health care services to all patients in collaboration with the other staff members to ensure that the individually planned needs and desires of patients are met, regardless of age or gender, on a continuing basis by performing the duties outlined below. The Nurse Practitioner will practice/perform the medical services as delegated or directed by the supervising physician as long as they are within the scope of practice for the RN/APRN. The NP will abide by the standards and laws governing the practice of nursing and advanced practice registered nursing as established by the designated State Board and specified State Professional and Occupational Standards. It is the obligation of each Nurse Practitioner to ensure that their supervisory requirements are adhered to, that the relationship of and access to the supervising physician is defined, and that a process for evaluation is established. It is the obligation of each Nurse Practitioner to keep current with required continuing education and licensure. 
 
Essential Duties and Responsibilities include the following.

<li>Maintains appropriate clinical standards. Follows the guidelines established by the Company policy and procedure manuals.</li>
<li>Examines patients for symptoms and/or physical information while ensuring the delivery of high quality, patient-focused health care all while recognizing the patient’s individual rights. </li>
<li>Aides the patients in their right to be informed and to participate in making decisions in all aspects of their medical care and treatment and initiates a patient education plan according to the individualized needs of the patient, including patient and family instruction. Ensures that all aspects of care have respect for personal values and beliefs, and is focused on the individual needs of each patient.</li>
<li>Elicits and reviews information about patient’s medical history.</li>
<li>Orders or executes various tests, analyses, results, reports and diagnostic images to provide information on patient’s condition in a timely manner to determine appropriate treatment and follow up. </li>
<li>Administers or prescribes treatments and medications appropriate for patient condition.</li>
<li>Promotes health by educational instruction as well as advising patients concerning diet, hygiene, and methods for prevention of disease.</li>
<li>Refers patients to medical specialist for consultant services when necessary for patient’s well-being.</li>
<li>Accurately, efficiently, and completely documents necessary data for patient records to facilitate completion of billing processes. </li>
<li>All entries into the medical record whether electronic or on paper will be legible, complete, dated, timed and signed with appropriate credentials. Documentation will be completed prior to discharge from the facility when possible but no later than 48 hours after discharge from the facticity. </li>
<li>Utilizes all equipment, supplies, facilities, and resources of the Company in a prudent and efficient manner.</li>
<li>Participates in department quality improvement activities, staff educational programs, attends departmental meetings, and demonstrates and maintains clinical competency in the delivery of patient care services in accordance with Company guidelines.</li>
<li>Participates in Peer Review and Chart Audit processes as designated by the Medical Director. </li>
<li>Maintains supervisory duties over staff as required.</li>
<li>Must maintain collaboration with the physician to deal with issues outside of their scope of practice.</li>
<li>Maintains competency in skills relevant to the scope of services provided in the facility. The scope of services in specific markets will require employees to complete relevant training and to obtain additional certifications or licensure. (DOT Certification, Occupational Medicine Certification, Dispensing Permits, Telemedicine permits, etc.)</li>

Job Description 
The Quality Systems Specialist (QSS) will be responsible for ensuring compliance with all internal Standard Operating Procedures and all pertinent regulatory and accreditation requirements. Duties include the following: perform QA review of organ and tissue donor records and associated data entry; collects, documents and distributes all relevant information to complete donor records and assure regulatory compliance; and ensures tissue processors and transplant centers are provided with all donor information required to help ensure the safety of the recipients of donated organs and tissues. Each level assumed the competencies of the essential job functions of the previous level.
This is a hybrid role that requires 1-2 days in office per week after a successful orientation period. There is a weekend coverage rotation once every 7/8 weeks.&nbsp;

<li>Collaborates with QSS team members to develop a staffing schedule that ensures QSS coverage 7 days a week (including holiday on call coverage).</li>

Bread for the City’s primary care clinic is a medical home that seeks to provide wrap-around medical, behavioral and social services to all medical patients. The social work team builds capacity for the medical clinic to better serve its clients by assessing social needs and providing care management services to medical patients. 
In addition, the Health Resource Room is a place where medical patients can go after they see their doctor to sign up for the patient portal, access health information and wellness programs, navigate the healthcare system, and obtain referrals to social service resources internally and externally. 
The Seasonal Social Services Specialist will support the social work team in providing care management and taking shifts in the Health Resource Room. 
Essential Responsibilities:

JOB SUMMARY
The Crisis Intervention Specialist serves primarily as an advocate and case manager for victims of sexual assault and provides hospital and law enforcement accompaniment to underserved and newly established hospitals and law enforcement agencies. The Crisis Intervention Specialist, along with other trained staff and volunteers support Resource &amp; Crisis Center of Galveston County, Inc. (RCC) Certified Rape Crisis Center programming and services. The Crisis Intervention Specialist responds in-person and by phone to victims of sexual assault, therefore this job requires access to reliable personal transportation to fulfill medical accompaniment responsibilities.
ESSENTIAL JOB FUNCTIONS

<li> Provide medical accompaniment on an on-call basis for victims of sexual assault.</li>
<li> Respond promptly to hospitals for medical accompaniment to victims of sexual assault.</li>
<li> Provide clients forms and information to begin recovery. Supports as advocate and facilitator working in partnership with survivors.</li>
<li> Help clients develop a recovery plan by jointly reviewing the assistance they have received, identifying any un-met needs, and determining possible sources for gaps in resources.</li>

YOUR NEXT OPPORTUNITY:
Under limited supervision, the Safety Specialist will implement and administer all aspects of the corporate environmental, health, and safety (EHS) program and the site-specific safety program for assigned area or responsibility.
WHAT YOU'LL DO:

Implement and administer environmental, health, and safety (EHS) program and site-specific environmental, health, and safety (EHS) plans.
</li>
<li>
Implement, administer, and conduct orientation and basic trainings. Continuously educating, coaching, and mentoring either in a classroom, meeting, or during job walks.
</li>
<li>
Performs regular and frequent site inspections and observations. Engages with employees, supervision, and management, continuously educating, coaching on safe work practices. Seeks opportunities to praise, recognize, and award our employees and teams.
</li>
<li>
Documents findings, compiles information, creates detailed reports, analyzes data, and tracks progress. Maintains files and records for long term storage.
</li>
<li>
Coordinates with client/owner, project management, field supervision, and divisional/regional leadership to ensure successful implementation of EHS program/plan. Represents safety in meetings.
</li>
<li>
Begins the shift in the field, coordinating with crews, participating in pre-task plans and stretch and flex, ensuring teams have what they need to work safely.
</li>
<li>
Performs, documents, and records environmental sampling utilizing tools like a dosimeter, light meter, multi-gas meter, etc.
</li>
<li>
Coordinates emergency response, provides care for injured employees, performs incident investigation, conducts interviews, compiles information and photographs, completes incident reports, participates in incident review process, tracks progress on recommended corrective actions, and develops lessons learned to be shared.
</li>

WHAT YOU BRING TO US:

Bachelor's degree in environmental, health, safety
</li>
<li>
High school diploma or equivalent w/ experience.
</li>
<li>
0-6 months with bachelor's degree
</li>
<li>
5 years of relevant experience w/ high school diploma or equivalent.
</li>

WORKING CONDITIONS:

General work environment - sitting for long periods, standing, walking, typing, carrying, pushing, bending. Work is conducted primarily indoors with varying environmental conditions such as fluorescent lighting and air conditioning

Noise level is usually low to medium; it can be loud on the jobsite.
