Patient Care Technician (PCT) 
Patient Care Technicians (PCTs) at Health Systems Management support and work under the guidance of RNs to provide dialysis treatment to multiple patients at a time. PCTs have a combination of technical skills and compassion needed to improve the quality of life for our patients while promoting positive patient outcomes. Successful candidates will have the opportunity to learn the discipline of dialysis. To be considered for this position qualified candidates are required to have a minimum of 6 months of healthcare and/or direct patient care experience. PCT Responsibilities and Physical Demands:
The physical demands and work environment characteristics described here are representative of those an employee encounters while performing the essential functions of the job.

<li>Set up of the hemodialysis machine. </li>
<li>Greet patients and collect details regarding their previous treatment. Report any concerns or observations to the lead nurse. </li>
<li>Assist with and prepare the patient's vascular access for treatment. </li>
<li>Initiate, monitor, and discontinue dialysis treatment per written policies, procedures, and standards of care. </li>
<li>Obtain patient's vital signs before, during, and after dialysis treatment and appropriately document in patient medical record. </li>
<li>Responsible for appropriately calculating and documenting patient's fluid removal. </li>
<li>PCT performs appropriate access cannulation and reports any difficulties or unusual findings to the lead nurse. </li>
<li>Monitor the patient for any adverse reaction(s) and conduct appropriate intervention.</li>
<li>Monitor the dialysis machine for any malfunction. Report any malfunctions to the lead nurse. </li>
<li>Demonstrates knowledge of infection control as related to hand washing, personal protective equipment (PPE), and universal precautions. </li>
<li>Maintain patient safety and comfort, which may include assisting with personal care needs and activities of daily living. </li>
<li>Responds appropriately to emergency situations. </li>
<li>Note you will have exposure to blood, needles, and various other bodily fluids. </li>
<li>This position provides direct patient care that regularly involves heavy lifting, moving of patients, and assisting with ambulation. </li>
<li>PCT will spend considerable time walking, bending, stretching, and standing. </li>
<li>The employee may occasionally be required to move machines, equipment, and may lift chemical and water solutions of up to 30 pounds and high as five feet. </li>
<li>You are required to acknowledge the inter-facility coverage policy that supports floating to various clinics during and after your training, if needed. </li>

Education Requirements and Position Qualifications:

<li>Basic computer knowledge required.</li>
<li>6 Months of healthcare and/or direct patient care experience is required.</li>
<li>Continued employment is dependent on successful completion of the Health Systems Management training program in its entirety. </li>
<li>The ability to work long hours is required. You could be on your feet for extended periods of time. Our day ends when patients’ care is completed.</li>
<li>You must have a flexible schedule and be able to work mornings, evenings, weekends, and holidays. </li>
<li>Must pass the Certified Clinical Hemodialysis Technician (CCHT) exam within 18 months following the first day of employment.</li>
<li>Must have or obtain Basic Life Support certification approved by the American Heart Association</li>
<li>Must be comfortable working around blood and needles, including cannulation, and obtaining labs.</li>

General Position Description:
The Nurse Practitioner assists physicians in overseeing the clinical care and treatment of patients and families; to consult with the patients’ attending physician, in consultation with the hospice physician; provide clinical direction to the Interdisciplinary Group/Team to ensure quality care. <li>When faced with two requests for one bed on a given unit, assures that the more clinically complex patient is placed on the “home” unit: the less clinically complex patient is overflowed to an appropriate unit.</li>
<li>Able to identify questionable diagnosis, and asks additional clinical questions so that the patient is assigned to the most appropriate unit and room type (negative airflow, isolation requirements, ICU vs. general care unit assignment, telemetry).</li>
<li>Performs pre-admission utilization management functions in accordance with established procedures.&nbsp;</li>
<li>Masters the use of the utilization management medical necessity criteria to determine appropriate level of care upon admission.</li>
<li>Monitors and assures clinical appropriateness of services and efficient use of the various levels of care and resources by working closely with members of the health care team, including the attending physician, house staff, and physician advisers on cases which do not meet established criteria.</li>
<li>Communicates/collaborates with external case managers and payers by providing accurate clinical information as needed.</li>

Actively Participates in Clinical Performance Improvement Activities related to Case Management Services:

<li>Assists in the collection and reporting of financial indicators including LOS, avoidable days, resource utilization, discharge barriers, cost per case, readmission rates, denial and appeals.</li>
<li>Uses data to drive decisions and plan/implement performance improvement strategies related to case management for assigned patients/units, including financial, clinical, quality and patient satisfaction data.</li>
<li>Collects data for discharge delays, over-utilization of resources, avoidable days and other data for specific performance and/or outcome indicators.</li>

Qualifications 
Required:&nbsp;

<li>Current license as Registered State of Illinois Professional Nurse or eligible for licensure in the State of Illinois.</li>

Bachelor’s Degree in Nursing and 2 years minimum experience in the acute care setting

<li>A high level of interpersonal skills to affect positive External Transfer outcomes.</li>
<li>Organizational skills necessary to prioritize and manage multiple external transfers at the same time.</li>
<li>Self-direction required for daily work.</li>
