Your Role:
<ul>
<li>Provide primary and family care evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning for our patients. &nbsp;</li>

<li>Conduct assessments, diagnoses, treatment plans, and health counseling</li>
<li>Perform administrative duties including clinic operations and patient follow-up</li>
<li>Deliver virtual care via secure telehealth platform with full EHR documentation</li>
<li>Collaborate with a multidisciplinary care team to ensure continuity of care</li>
<li>Work 30 hours in clinic and 10 hours of telehealth in remote setting.</li>

Required Qualifications:

<li>Active, unrestricted FNP license active state license</li>
<li>Minimum 1 year of NP (2+ years preferred)</li>
<li>BLS certification and ability to pass respirator FIT test</li>
<li>Willingness to obtain multi-state licensure (for telehealth roles)</li>
<li>Proficiency in EHR (EPIC preferred) and strong communication skills</li>
<li>Broadband internet and private, professional remote workspace (for telehealth)</li>

Your Role
After completing a 14-week training and mentorship program, you will:

<li>Responds to emergencies and makes logical decisions regarding non-formulary drugs or off-site referrals. </li>
<li>Supports existing policies and procedures for admission, transfer, and specialty consultant referrals. </li>

Qualifications
